Instruction: To All School Head the first page is for the teachers to comply in duplicate copy but validated by the
school head
the 2nd page will comply by the school head which I will be the one to validate. The hard copy shall be submitted
to New Alimodian January 28. All needed data shall be filled up. One copy for the district and one copy for the
school.
This is also your MOVs for Instructional Supervision as required by the CID-- SCHOOL HEAD must have to conduct
IS/TA 10 teachers per month as required it depends on you if you are going to add. But for my report only 1 per
teacher and 1 for school head. For the District Consumption this forms shall be filled up only after every end of
quarter but for school head monthly monitoring for IS/TA of not less than 10.
Validation of SH is to inspect if the said answers are true or false then summarized the report  in your school head
template
