CONTRACT OF SERVICES

This Contract for Service is entered into by and between the DepEd, Division Office of Masbate,
represented by NENE R. MERIOLES, CESO V, Schools Division Superintendent and DENIA C. ACER of
legal age and a resident of SALVACION, BALUD, MASBATE.

WITNESSETH

1. That DENIA C. ACER agrees to devote his/her technical know-how, experience, and skills to the agency
and other related work thereto at the rate of THREE THOUSAND FIVE HUNDRED PESOS ONLY
(Php 3,500.00)/month of actual services, (excluding Saturdays, Sundays and holidays).
2. That it does not entitle him/her to payment of PERA/COLA, RATA, and other benefits being enjoyed by
government employees.
3. That it is agreed that the second party is subject to the conduct of random drug testing per existing and
applicable policy of the Department of Education; is under obligation to be drug-free for the duration of the
contract, and that any confirmatory positive result from a random drug testing program of DepEd, unless
negated in a challenge test on the same sample, shall result in the automatic pre-termination of the contract
of service or job order.
4. Payment herein provided shall be subject to the usual accounting and auditing rules and regulations.
5. That this contract shall take effect on JULY 01, 2022, and ceases to be in force and in effect upon the
determination and notice by the agency of unsatisfactory performance of work or for such other causes
authorized by law but not beyond DECEMBER 31, 2022.
6. That I am not related within the 3 rd degree of consanguinity and/or affinity with the appointing authority of
the hiring agency.

Received and held myself bound by the terms, conditions and stipulations of this contract and
other applicable rules and laws.
