EXHIBIT “A”
GENERAL INSTRUCTIONS
1.0 APPROVED VENDOR LIST:-
COMPANY’s approved sub-vendor / manufacturer list is in Exhibit “B” [COMPANY’s Approved

Manufacturer’s List]. SUPPLIER should follow the same for the procurement of the item included in
the package or in the supplied equipment.
2.0 SUB-SUPPLIERS / SUB-CONTRACTORS:-
All sub-suppliers / sub-contractors that the BIDDER proposes to use in the performance of the Work as
well as the approximate value of that portion of the Work must be listed in Exhibit “C” [List of Sub-
Suppliers / Sub-Contractors] of the BIDDER’s proposal. COMPANY reserves the right to review, and
accept or reject any or all of the BIDDER’s proposed sub-contractors.
3.0 SPARE PARTS:-
3.1 BIDDER should submit a separate price list for start-up & commissioning spares, insurance
spares & 2 years operational spares along with the bid as per Exhibit - 2 [Spare Parts
Requirements].
3.2 Spares Parts bid validity should be minimum ninety (90) calendar days more from date of
issue of Purchase Order (PO) for the main equipment. Any PO shall be issued within the period
of applicable bid validity. BIDDER to make sure that the unit prices for the spare parts shall
cover the extended bid validity as per 3.2.
4.0 VENDOR ASSISTANCE IN INSTALLATION & COMMISSIONING:-
4.1 SUPPLIER have to provide the assistance in installation, commissioning & start up and training
on the equipment as required in Exhibit - 5 [Supplier Data Requirement & Schedule].
4.2 SUPPLIER has to provide the detail schedule for installation, commissioning & startup
preferably level 5 schedule. Schedule shall also include the time period required for
SUPPLIER’s site engineer & representative.
4.3 Unit Rates should be quoted for these services along with the bid.
4.4 COMPANY will inform the SUPPLIER in advance to assign their personnel to assist in site
activity for commissioning of the equipment.
4.5 The PO award will be contingent on SUPPLIER’s agreement to support installation,

commissioning, and start-up activities.
5.0 EXCEPTIONS TO TECHNICAL REQUIREMENTS:-
5.1 COMPANY purchase order(s) resulting from this REQ shall also be based on the technical
requirements of this REQ.
5.2 If BIDDER must take exception, BIDDER shall specify clearly the exception(s) in Exhibit - 9
[List of Technical Exceptions] of the bid, explain specifically what is not acceptable and
BIDDER is to justify why specific technical exception been taken and propose alternate with
proper technical justification.

6.0 ALTERNATE PROPOSAL:-
6.1 If BIDDER wishes to propose alternate technical and/or commercial proposals to the base bid,
only alternate proposals by the Original Equipment Manufacturers (OEMs) identified on the
Exhibit “B” [COMPANY’s Approved Manufacturer’s List] shall be acceptable. Alternate
proposals shall be consistent with a “like for like in function” philosophy.
6.2 If BIDDER wishes to propose alternate technical and/or commercial proposals to the base bid,
BIDDER may do so by attaching such proposals, in this the BIDDER shall state price and
schedule impact of the alternate proposal. Alternate proposal shall not deviate from the
specifications. Also BIDDER shall state reasons why the alternate will be advantageous to
COMPANY.
6.3 BIDDER shall note and be responsible for accounting for any and all impact of their alternate
proposals would have on all options requested by COMPANY in any portion of this REQ. Any
impact/change not noted by BIDDER shall obligate them to bear the contract should the
COMPANY elect to accept the alternate proposal.
7.0 DOCUMENTS SUBMISSION:-
7.1 Documents required to be submitted are listed in Exhibit - 5 [Supplier Data Requirement &
Schedule]. BIDDER may add the other required document as applicable.
7.2 BIDDER should also complete the data sheets and other project document as listed in Exhibit
- 7 [Project Drawings, Datasheets, Specifications Etc.].
7.3 The documentation procedure to be followed by BIDDER is listed in Exhibit - 4

[Documentation Procedure] and BIDDER has to follow it strictly for any document submittal
in the event of PO award.
8.0 VENDOR QUALITY PLAN:-
8.1 Minimum requirement of Quality Assurance (QA) plan to be followed by BIDDER is listed in
Exhibit - 10 [Quality Plan].
8.2 BIDDER should follow the proper quality plan for specific items and submit the quality plan as
per Exhibit - 10 [Quality Plan].
8.3 If the BIDDER wishes to submit their own quality plan, such plan shall meet COMPANY’s
requirements as stipulated in Exhibit - 10 [Quality Plan].
9.0 EQUIPMENT NAME PLATE & TAGGING:-
9.1 All major equipment should have the name plate with equipment tag number etc.
9.2 All bulk material and loose items as a minimum should be tagged properly for correct
identification and erection site.

SPARE PARTS REQUIREMENTS
START- UP & COMMISSIONING SPARE PARTS:-
1.1 Those parts required to prepare the equipment for start-up, and to safeguard the operation of
equipment during storage, installation, pre-commissioning, commissioning, and start-up periods.
1.2 The selection shall be based on the BIDDER’s recommendations and shall be reviewed and
revised by COMPANY as required.
1.3 BIDDER should submit a Lump Sum price in Item No. 9, Exhibit - 1 [Schedule of Quantity]
and separate price list for all the parts with the required quantity and unit price along with the
bid.
2.0 INSURANCE SPARE PARTS:-
2.1 Those parts of equipment, equipment assemblies, or complete items of equipment that are
required for replacement of items not subject to deterioration by normal use, but failure of which
is critical for continued and safe operation of the equipment or plant after the start-up period.
2.3 BIDDER should submit a Lump Sum price in Item No. 10, Exhibit - 1 [Schedule of Quantity]
and separate price list for all the parts with the required quantity and unit price along with the
bid.
3.0 TWO (2) YEAR OPERATING SPARE PARTS:-
3.1 Those parts of equipment, equipment assemblies, or complete items of equipment that is
recommended and required to maintain the operation of equipment during the first two (2) years
of operation.
3.3 BIDDER should submit a Lump Sum price in Item No. 11

Notes:-
3.1 BIDDER is to include in the offer an estimated cost and schedule (BIDDER to state clearly the required
number of days on site and travel days) for the assistance in Installation, Commissioning, Start up and
Training of the equipments as required in Exhibit - 5 [Supplier Data Requirement & Schedule]. Unit
Rates will be used for such services. These services may require the physical presence of suitably
experienced VENDOR staff on site during Installation, Commissioning and Start-up of the plant. Test

deemed necessary and should COMPANY require that the VENDOR provide test equipment.
3.2 BIDDER to confirm the respective lump sum cost for the return flight from his location (country of origin)
3.2.1 For Expat Personnel from outside Nigeria: To & from Logos which shall include the travel days,
necessary accommodation on arrival day and necessary accommodation on departure day
including and any other local transportation from/to his accommodation to/from Airport vice versa.
This includes all travel documentation e.g. Visa, etc.
3.2.2 For National Personnel from in county Nigeria: To & from Logos which shall include the travel
time, necessary accommodation on arrival day and necessary accommodation on departure day
including and any other local transportation from/to his accommodation to/from Airport vice versa.
3.3 The applicability of the day rate as follows:

3.3.1 For Expat Personnel: The day rate shall commence from the day the person arrives at Chevron
nominated Airport (either Aero Contractors Airport or Local Airport - MM2) for the further travel to
Chevron field or site locations and the day rate cease from the day person arrived at Chevron
nominated Airport (either Aero Contractors Airport or Local Airport - MM2) for his further travel to
his home country.
3.3.2 For National Personnel: The day rate shall commence from the day the person arrives at
Chevron nominated Airport (either Aero Contractors Airport or Local Airport - MM2) for the travel
to Chevron field or site locations and the day rate cease from the day person arrived at Chevron
nominated Airport (either Aero Contractors Airport or Local Airport - MM2) for his further travel to
his home in country.

SUPPLIER DATA REQUIREMENT & SCHEDULE
COMPANY requires that the BIDDER submit a complete Supplier Data Book(s) and Equipment Manual(s) as per
the below Supplier Data Schedule as a MINIMUM. This data is required to support completion of Purchase Order
requirements and COMPANY places great importance on timely receipt of accurate and useful “as-built” supplier
data. The approved supplier data schedule will become part of the Purchase Order if the event of award.
Notes:-
1. The payment will be made after the acceptance of all Drawing and Data Requirements as listed in the
Exhibit - 5.
2. All documents / drawings and data shall include Customer, Item Number, Project, & Purchase Order
Number.
3. Transmittals shall be clearly marked with the Customer, Project & Purchase Order Number.

QUALITY PLAN
1. COMPANY plans to maintain an inspection staff throughout the duration of the manufacturing / assembly
in order to independently monitor the SUPPLIER’s QA/QC activities.
2. BIDDER shall provide with the bid a brief description and organizational chart which identifies SUPPLIER’s
QA/QC activities during the course of the project. The description shall describe the interfaces between the
SUPPLIER’s internal organizational groups and COMPANY’s engineers and inspectors. The description shall also
identify QA/QC activities of SUB-SUPPLIER and their interface with SUPPLIER. All personnel involved with the
QA/QC activities shall be identified in the bid and certification from outside agencies shall be listed.
3. In addition, the SUPPLIER shall complete and submit with the bid, a project specific Quality Plan which is
similar in format to the below sample as a MINIMUM. The Quality Plan shall indicate project controls of SUPPLIER
who shall use from the purchase order award through the final delivery of the equipment. The Quality Plan shall
identify all SUPPLIER QC Hold / Witness / Review points, including sub-contracted work, and all COMPANY’s
Hold / Witness / Review points in line with Exhibit - 11 [Inspection & Test Plan].

INSPECTION & TEST PLAN
Minimum requirement of COMPANY’s Inspection & Testing Requirements (ITR) are listed in this Exhibit - 11.
BIDDER has to review carefully & to confirm compliance. If BIDDER must take exception, BIDDER shall specify
clearly the exception(s) in Exhibit - 9 [List of Technical Exceptions] of the bid. The ITR is the basis for developing
SUPPLIER / SUB-SUPPLIER (S) Inspection and Test Plan (ITP).
BIDDER shall complete the ITP and submit with the bid which is similar in format to the attached sample in this
REQ.
The ITP shall cover the major quality-related activities in chronological order from contract review, through
manufacturing / fabrication, assembly to final testing, documentation, and certification. If the BIDDER wishes to
submit their own ITP, such ITP shall meet COMPANY’s ITR requirements as a minimum and the following
information as minimum is included.
1. Locations at which each activity will take place, including SUB-SUPPLIER premises.
2. Quality verification activity / stage.
3. Reference document / procedure or method statement to perform activity, including specific

reference to actual section / page of procedures, standards, instructions, specifications, etc.
4. Acceptance criteria.
5. Certifying or verifying documents generated to provide evidence of compliance with specified

requirements.
6. CONTRACTOR, Third Party and COMPANY inspection activities defined in terms of Hold /
Witness, Observe, Document Review and Inspect points.
Where activities are to be undertaken by a SUB-SUPPLIER (S), the ITP shall make reference to this fact and
include details of SUB-SUPPLIER (S) involvement. Separate ITPs shall be required for each SUB-SUPPLIER’s
work.
All inspection and testing activity shall be performed using COMPANY’s accepted ITP, Procedures, and
acceptance criteria. All ITPs shall be accepted by COMPANY prior to commencement of any work.
SUPPLIER shall develop all test procedures and submit to COMPANY for review and acceptance - including any
/ all SUB-SUPPLIER (S) testing procedures.
SUPPLIER shall ensure that all applicable codes, standards, and relevant acceptance criteria documents are
available at the work location, and that COMPANY’s representatives on site shall have ongoing access to this
information.
SUPLLIER shall be ultimately responsible for the development and proper implementation of all ITPs, including
those reviewed or developed by SUB-SUPPLIER (S).
COMPANY reserves the right to select witness and hold points within all developed ITPs for COMPANY’s oversight
of selected functions and to perform surveillance or audits of the Work.
THIRD PARTY INSPECTION
COMPANY shall arrange for any third party inspection of equipment, materials, installation, erection or
construction required by statutory regulations. The SUPPLIER shall give COMPANY at least 10 Working days’
notice of all visits to the Work site(s) by third party inspectors, to plan COMPANY’s participation in such visits.

COMPANY may inspect and shall have free access at all times to any part of SUPPLIER’s / SUB-SUPPLIER’s
facilities associated with COMPANY Work. COMPANY reserves the right to participate in any audits scheduled
by SUPPLIER / SUB-SUPPLIER or to plan and conduct its own audits including SUPPLIER / SUB-SUPPLIER
facilities. COMPANY shall have the option of inspecting every item and procedure associated with the Work.
Work shall not proceed beyond COMPANY inspection "witness" or "hold" points without COMPANY inspection
or COMPANY written waiver of inspection. COMPANY inspectors shall have the right to explore any apparent
defects in Materials, welds or fabrication. SUPPLIER / SUB-SUPPLIER shall provide office space, drawings,
phone and other support Materials for COMPANY inspection personnel during execution of the Work. In addition
to inspection points indicated on the ITPs, COMPANY may, at its sole option, inspect any activity related to
COMPANY’s purchase order in SUPPLIER’s / SUB-SUPPLIER’s facilities at any time. SUPPLIER / SUB-
SUPPLIER shall fully support COMPANY in this activity on an accompanied or unaccompanied basis as required
by COMPANY.
Methods of COMPANY inspection may include, but are not limited to: visual, metallurgical, dimensional, or any
suitable non-destructive testing method. The type and extent of inspection shall be at the discretion of COMPANY.
SUPPLIER shall provide five (5) working days advance notice for U.S. domestic travel by COMPANY, or ten (10)
working days notice for international travel by COMPANY prior to the start of any COMPANY Required Notification
for “Hold/Witness” or “Observe” inspection and test points. For performance or mechanical tests, SUPPLIER /
SUB-SUPPLIER shall provide pre-witness test data by FAX or e-mail at the time of final confirmation/notification
of the test date.
For any COMPANY identified defects not corrected in a timely manner by SUPPLIER / SUB-SUPPLIER, the
SUPPLIER shall provide a Non-conformance Report which provides the reason for non-conformance and
corrective actions to be taken. Based on applicable codes, standards and specifications, COMPANY shall be the
sole judge of the acceptability of such non-conforming conditions and corrections, and COMPANY decisions shall
be final. The SUPPLIER Non-conformance Reports, that provide a disposition action other than acceptance based
on the original acceptance criteria, shall be accepted by COMPANY prior to proceeding with remedial action.
Where COMPANY inspectors have just cause to suspect the quality of any item, the inspector may require that
additional inspection or testing be performed either by SUPPLIER / SUB-SUPPLIER or by a third party selected
by COMPANY at no extra cost to COMPANY. Prior to this additional inspection or testing, the inspector will so
advise COMPANY or designated representative of the situation.
SUPPLIER shall ensure that all engineering and inspection documentation is readily available to COMPANY
inspectors at SUPPLIER’s / SUB-SUPPLIER’s facility at all times. COMPANY shall have complete access to all
documentation.
SUPPLIER to submit the following documentation:
1. A Project Quality Management Plan aligned with ISO 10005 Quality Management Systems Guidelines
for quality plans.
2. Records of material tests, including PMI, are to be available for review by inspectors at the time of
inspection and are to be submitted at the time of final inspection (documentation to accompany
shipment or otherwise be available for receiving inspection).
3. Welder qualifications and welding procedures that will be used in the performance of Work shall
require COMPANY’s review and acceptance prior to commencing manufacture. Welders shall be
suitably qualified. The requirements for qualification shall be specified and welder records shall be
maintained by SUPPLIER.
4. Special installation procedures and other required fabrication or manufacturing procedures (i.e. those
required for post weld heat treatment, tube rolling, coatings, etc.) that will be used in the performance
of Work shall require COMPANY’s review and acceptance prior to commencing manufacture.
Personnel carrying out special processes (e.g. NDE, welding, coating, and heat treatment) where the
results cannot be fully verified by subsequent inspection and test shall be suitably qualified. The
requirements for qualification shall be specified and personnel records shall be maintained.
5. Instructions and requirements for equipment and materials storage, preservation, and maintenance,
including identification of materials required for preservation and maintenance, are to be provided

sufficiently prior to receipt (prior to shipment or earlier) to ensure appropriate resources are available
at the time of delivery.
6. Copies of hazardous area third party certificates of testing for every electrical and instrument device
supplied that is required to meet a hazardous area classification.
