The undersigned Resident(s) of the apartment dwelling described above hereby give notice of intent to move-out on or before
the date noted above. The undersigned Resident(s) are aware that the refund of the security deposit(s) is subject to
compliance with the terms and conditions of the Lease Agreement, including:
• Forwarding address. Please provide your new address in writing to the leasing office on or before move-out.
• Key/entry device return. Return all apartment, mail, storage, community keys and entry devices on or before move-out
date.
• Repairs. Clean and repair premises other than conditions caused by normal wear and tear.
Resident(s) will be supplied with Move-Out Cleaning Instructions and should request a joint inspection with a community
representative at the time of move-out.
Multiple Resident: If multiple Residents are on the lease, notice from one will be considered notice from all.
Failure to Move Out: Six Oaks and potential new Residents rely on the accuracy of this move-out notice. Six Oaks may
clean, repair and re-let the apartment for occupancy on the day after the above move-out date. It is acknowledged that if any
of the undersigned Residents hold over and fail to move out on the above move-out date:
• There may be security deposit deductions relating to holdover, as authorized in the lease; and
• Residents are subject to all contractual and statutory remedies for violation of the lease, including damages, attorney fees,
late payment charges, rent acceleration, cost of re-letting, utility cut-off, and in the event of unauthorized holdover, liability
for increased holdover rents and one month lease extension.
Responsibility for Rent: If the move-out date noted above precedes the lease expiration date noted above, or if written notice
is less than twenty (20) before the move-out date, Resident is subject to all contractual and statutory remedies including rent
acceleration and cost of re-letting fees.
Security Deposit Notice: The resident(s) security deposit will be applied to any unpaid charges at move out.
Utilities: Final utilities are billed in arrears. Resident(s) will be charged for the final balance after move out.
The security deposit may be applied to cover final utilities if applicable.
