SAFETY POLICY
• Provide adequate control of the health and safety risks arising from
our work activities

RESPONSIBILITIES
Management Responsibilities
The Management of Smartflow is responsible to providing employees
with a safe healthful workplace by:
Employee/Staff Responsibilities
It is critical to the success of our HSE program that employees have the
concept as their goal that all injuries and incidents can be prevented.
Senior Management
• Provide the necessary resources to implement, support, and enforce the
health and safety policy and program within the company
• Promote the exchange of health and safety information
• Review accident reports and communicate with concern authorities
regarding notices respecting critical injuries, accidents, incidents and
other events
• Provide support for the Health & Safety Program and any safety related
initiative (i.e. health & safety targets/goals)
Supervisor/ Manager
• Review workplace safety inspection checklists and respond to
recommendations
• Discuss matters of safety with workplace H&S representative and review
& respond to recommendations
• Coordinate workplace safety meetings
• Establish a workplace health & safety documentation system for
workplace/project safety inspection checklists, injury reports, Material
Safety Data Sheets, emergency & rescue procedures and other
documents
• Perform daily workplace tours, noting any hazards or unsafe practices
• Respond immediately to hazards and keep records for follow-up
• Identify special workplace hazards and outline appropriate safe work
procedures and training requirements
• Arrange necessary safety training to all employees.
Worker
• Work in accordance with the Occupational Health & Safety Act (OHSA)
and workplace Regulations
• Participate in workplace inspections when required
• Implement workplace inspection and safety meeting recommendations
• Report hazardous conditions or contraventions of the OHSA and
workplace Regulations to the Health & Safety representative or
supervisor
INCIDENT INVESTIGATION
Guidelines
Investigating incidents and accidents is a critical process that provides
insight into the contributing factors and allows decisive action to reduce
future risks. All incidents that result in injury or property damage or that
could have resulted in serious injury or property damage (near miss)
must be reported and thoroughly investigated.
Senior Management
• Ensure training is provided for Supervisors and Health & Safety
Representatives in Accident Investigation and First Aid
• Receive and review Accident Investigation reports
• Initiate action deemed necessary as a result of the investigation
Supervisor
• Supervisor at the site/workplace where incident occurred will perform
an incident investigation
• Review the workplace and conditions to understand the accident
situation
• Receive reports of accidents, injuries and incidents and encourage the
reporting of these to all employees
• Implement temporary control measures to prevent any further injuries
to employees or damage to equipment/assets.
• Ensure that the injured person has received or is receiving adequate
medical aid and appropriate transportation of the injured person to
medical treatment has been arranged
• Investigate casual conditions and unsafe acts; make conclusions based
on existing facts. Complete the incident investigation report with
recommendations for corrective actions
• Complete accident investigation with the Health & Safety Representative
• Distribute copies of the Accident Report and Accident Investigation to
the Senior Management along with all employees
Worker
INCIDENT REPORTING
Supervisor
• Review all hazard reports
• Take corrective action to reduce risks of injury from hazards identified in
hazard reports
• Discipline workers who knowingly disregard safety rules to expose
themselves or others to hazards
• Submit all records of orientation, training, safety meetings and hazard
reports to management, with a copy available on the workplace
• Ensure all accidents or hazards requiring investigation are reported
immediately to head office
• Ensure that H&S representative are consulted on recommendations
Worker
• Report all hazardous conditions immediately to their supervisor
• Participate in investigation of hazard reports, providing information into
the cause and elimination of the hazard
Housekeeping
Guidelines
Proper housekeeping procedures are essential to an effective Health &
Safety Program. A successful H&S program is proven to reduce or
prevent accidents, injuries and illnesses and ultimately, save lives.
Consistent with company policy, Smartflow will not tolerate an untidy
workplace. It is the responsibility of all staff/subcontractors to clean
their area of work daily, or as often as necessary to maintain a clean
standard.
When subcontractors‟ waste and scrap create a hazard and are not
cleaned up in a reasonable time, it will be cleaned up by Smartflow at
Housekeeping Responsibilities
Supervisor
• Arrange for work areas to be cleaned up at least daily
• Inform all staff/workers about waste storage location and provide
adequate resources to clean waste on daily basis
• Review housekeeping problems and requirements in safety meetings
with all workers
• Ensure that all waste and scrap that might create a hazard is cleaned up
and/or secured
Worker
• Perform daily clean up of work area
• Store materials safely and in an organized manner
• Dispose of waste and scarp at provided waste storage location
• Bring housekeeping related hazards to the attention of the H&S
representative, supervisor and other workers on the workplace
Housekeeping Procedures
Daily cleanup of all work areas, disposal of scrap as procedure provided
by Smartflow
• Visually inspect work areas at the beginning and end of each shift
• Regular remove of scrap, material and garbage from workplace garbage
bins
• All walkways, stairways and passageways must be clear of materials,
tools, cords, garbage etc. at all times
Storage
Flammable Materials
• Use copper grounding straps to reduce static electricity build up
• Store fuels in containers approved and stamped
• Ensure electric fixtures and switches are explosion-proof where
flammable materials are stored
Hazardous Chemicals
• Refer to Material Safety Data Sheets (MSDS) for specific safety and
storage information on each controlled substance
• Follow manufacturer’s recommendations for storage
• Comply with all restrictions concerning environmental factors and
minimum distance requirements
• Post legible warning signs where required
• Provide suitable equipment for prompt spills response where required
• Store empty containers in a secure location, apart from full containers
RISK MANAGEMENT
CONTRACTOR MANAGEMENT
• First aid reports refers to an injury that can be treated on the job
without any work days lost
• Medical aid reports refers to an injury not severe enough to warrant
more than one day off work, but medical treatment is required
• Lost time injury reports refers to an injury that direct result keeps an
employee off work for more than one full day
• Equipment maintenance records
• Joint Health & Safety Committee meetings
Supervisor
• Record all accidents, incidents, first aid occurrences, lost time injuries,
equipment damage, MOL reports and JHSC minutes and make available
to all on the workplace
• Send all relevant health and safety information to the Smartflow head
office
• Coordinate first aid response, accident investigation or other follow up
procedures subsequent to an occurrence
• The Supervisor will control and maintain all required records. Records
are maintained for a minimum of Three years following the end of the
year to which they relate.
Worker
• Report all accidents, incidents, first aid occurrences, lost time injuries
and equipment damage to supervisor
Supervisor
• Ensure complete orientation is provided for all new workers on a
workplace
• Ensure completed checklists for new worker orientation are available on
the workplace
Contractor
• Provide project orientation to all members of a crew prior to beginning
work on a project
• Submit completed orientation checklists to the supervisor
Work Refusal
Guidelines
According to the Section 43 (3) Occupational Health & Safety Act,
A worker may refuse to work or do particular work where he or she has
reason to believe that,
(a) Any equipment, machine, device or thing the worker is to use or
operate is likely to endanger himself, herself or another worker;
(b) the physical condition of the workplace or the part thereof in which
he or she works or is to work is likely to endanger himself or herself;
(c) Any equipment, machine, device or thing he or she is to use or
operate or the physical condition of the workplace or the part thereof in
which he or she works or is to work is in contravention of this Act or the
regulations and such contravention is likely to endanger himself, herself
or another worker.
All workers have the right to refuse is they believe a hazardous condition
exists in their workplace. The worker must notify his supervisor
immediately. Before this becomes a work refusal under the
Occupational Health & Safety Act, the worker and supervisor should
discuss the hazardous condition to determine if there is issue can be
resolved internally. If the issue cannot be resolved, and the worker still
feels that a hazardous condition exists, the Management will be involved
to determine if the condition is likely to endanger. An investigation will
be performed to identify corrective action needed to achieve a
resolution. Education can be included as corrective action. Once the
corrective action is implemented, and all parties agree that the
hazardous condition has been resolved, work can continue.
Supervisor
• Provide orientation to all workers including a review of their right to
refuse hazardous work
• Support all workers‟ right to refuse work they fell may be hazardous
• Investigate the report of hazardous work immediately
• Attempt to resolve the hazardous condition internally
• Adhere to the work refusal process if it cannot be resolved internally
with a JHSC member, a H&S rep, and/or worker representative (see
Work Refusal Flow Chart in Checklists & Forms section)
• Ensure the work considered hazardous is not resumed until
recommendations are implemented and the resolution is accepted by all
parties
• Never threaten or impose reprisals on workers who refuse work
• Ensure communication exists amongst all parties during a work refusal
Worker
• Report hazardous conditions immediately to direct supervisor
• Assist in attempting to resolve the hazardous condition internally
• If no resolution is acceptable, continue to refuse to work and stand by
while investigation is performed
• Resume work once corrective action is implemented and a resolution is
achieved
Company rules-
You must follow these rules while you are on company premises and while
you conduct company business. These rules apply any place you conduct
company business.
1. Using, possessing or being under the influence of intoxicants or illegal
drugs on company premises or while conducting company business is
strictly forbidden.
2. The possession of deadly weapons or explosives on company premises
or while conducting company business is strictly forbidden.
3. You may not sell, buy, transfer or distribute drugs. We reserve the right
to report such actions to the authorities.
4. You must cooperate with any investigation into drug or alcohol use. An
investigation may include tests to detect the use of alcohol, drugs or
inhalants.
Responsibilities-
Management
• Responsible for seeing that fire-prevention procedures are established
and enforced
• Fire suppression systems are inspected regularly and maintained
• Employees are trained to use fire extinguishers for incipient fires
• Employees are trained to use evacuation routes and procedures
Supervisors
• Responsible for monitoring the use of flammable materials
• Training employees in safe storage, use and handling of flammables
• Ensuring that storage areas for flammables are maintained properly
Employees
• Responsible for following company procedures for the safe storage, use
and handling of flammable materials
• Reporting violations of the Smartflow Company fire prevention plan
RESPONSE PROCEDURES
• If possible and not a risk to life, isolate all fuel sources and/or threatened
facilities.
• Fighting a fire in the initial stages is considered incipient fire fighting. Do
not fight fires beyond the incipient stage unless you are trained and
equipped to do so as a part of the fire team. Such fire fighting should be
limited to trained personnel using fire extinguishers, fire hose and water
monitor streams at long range.
• Locate the fire fighting equipment and approach the fire from the
upwind side. In the case of a gas fire, extinguish the fire by shutting off
the fuel source, where possible.
• Never pressurize an extinguisher in such a manner that any part of the
body is located directly above or in front of the fill cap or under the base
of the extinguisher.
• After the fire is extinguished, stand by to ensure that there are no
flashbacks. Never turn your back to a fire, whether or not it appears to
be completely extinguished.
FIRE EXTINGUISHERS
Portable fire extinguishers can be very effective for fighting fires in their
incipient stages. A person who is well-trained in fire-extinguisher use can save
both lives and property. Portable fire extinguishers must be available even
when other fire fighting measures are available. For extinguishers to be
effective in a fire situation, proper selection, inspection and maintenance are
essential.
Make sure all fire extinguishers are placed in conspicuous locations, clearly
visible and easily accessible. Keep all fire extinguishers fully charged and
operable, and in their proper locations at all times.
Supervisors and safety committees are responsible for work site inspections to
ensure compliance with the company Fire Safety Program. These inspections
should address housekeeping issues, proper storage of chemicals, access to fire
extinguishers and emergency evacuation routes.
Operational Hazards
1) Walkways , emergency escape route and working areas shall be
kept clean and free of obstractions.
2) Good housekeeping shall be maintained in all areas ( this
includes offices and site)
3) Compressed air and others gases under pressure must be used
only for the purpose intended.
4) Do not operate machinery or equipment unless you are
authorized to do so.
5) Use the right tools for the job.
6) Inspect tolls regulary for damage and defects. Replace or retrun
all defective tools.
7) Never use a portable electrical tools unless it is in good
conditions and certified for use.
