INCIDENT REPORT:
I am writing to you to report a person with COVID-19 symptoms named as Ms. Maria Carlina Agcaoili,
an architect of Agcaoili Group.
The incident occurred on November 14, 2022 at 9:30 in the evening when I, along with the barangay
police officer named Joshua Padilla and police mobile driver Sevi Estarez, witnessed Ms. Agcaoili
walking down the street of Mendiola amidst the past curfew, coughing and having chills.
The involved person, Ms. Agcaoili, was walking in Mendiola street due to unavailability of vehicles
because it is past curfew. Ms. Agcaoili admitted that she has been having a fever and chills since
November 11, 2022. Also, she admitted that in the morning of November 14, 2022, she lost her sense
of taste and started having a difficulty in breathing and yet, she still managed to go to work and finish
some paper works. We bought Ms. Agcaoili to the nearest COVID-19 facilities that our Municipality
lends for possible COVID-19 positives.
However, inside the police mobile, we notice a rash on her skin and a discoloration in her fingertips.
With these noticeable symptoms, the chances of her being a positive of COVID-19 had increased.
Consequently, I would like to recommend a RT-PCR Test to be conducted immediately and start
contact tracing for the people who have been in contact with her for the past 7 days. I would also like
to recommend that you also quarantine us because we are also part of the people she’s been in contact
with. Also, I would like to seek the attention of our Municipality Mayor, Atty, Kallix Joel Gatchalian,
regarding the possibility of the first person in Costa Brava acquiring a COVID-19 virus. I would like to
ask for immediate response, if ever Ms. Agcaoili turns out to be positive, by a municipal lockdown to
further minimize the risk of spreading infection.
