CONTRACT OF HAULING SERVICE AGREEMENT

KNOW ALL BY MEN THESE PRESENTS:

This Agreement made and entered into Bacolod City, Philippines, by and between.

JUVY S. TALIMODAO PhD, of legal age, Principal IV, of FR. GRATIAN

MURRAY, AFSC INTEGRATED SCHOOL, DepEd Division of Bacolod City
hereinafter referred as the FIRST PARTY.

-and-

FRANCISCO TIBALAN RAGO JR, of legal age and a resident of Hda,

Carmen ,Barangay Granada,, Bacolod City, Negros Occidental, and the
owner/operator of one (1) TRICYCLE (vehicle description) hereinafter referred
as the SECOND PARTY.

WITNESSETH:

WHEREAS, the FIRST PARTY is desirous in engaging the services of the SECOND
PARTY in the hauling of the bottled milk from Andres Bonifacio Elementary School I to
Fr. Gratian Murray AFSC, Integrated School, Crossing Carmen, Barangay Granada,
Bacolod City under the School Based-Feeding Program-Milk Component FY 2022;

NOW TEHREFORE, for and in consideration of the foregoing premises the parties
hereto have agreed as they do hereby agree, as follows:

1. That the SECOND PARTY will provide one (1) vehicle to haul the bottled
milk from Andres Bonifacio Elementary School I to Fr, Gratian Murray, AFSC
Integrated School once a week for the three (3) weeks;

2. That the FIRST PARTY shall pay the SECOND PARTY the amount of
SIX HUNDRED PESOS ( Php 600.00) PESOS per hauling for three (3) weeks
the duration of the distribution of the bottled milk under the School Based-
Feeding Program-Milk Component FY 2022 or the total amount of One
Thousand Eight Hundred Pesos only (Php 1,800.00) as payment for the hauling
services it will render.
