Greetings of Peace!

May I have the honor to request from your good office the monetization of my leave credits
due to the following reason to wit:
a) My husband is a Stage 5 Chronic Kidney Disease or dialysis patient since October 2021 up
to the present. He has no work and all the expenses is coming from my salary.
b) He has an outstanding balance at Socsargen Hospital admitted last April 30-May 17,2022
in the amount of P60,000.00.
c) He was also admitted at Mindanao Medical Center last May 24-June 10,2022.
d) Presently, he is in Mindanao Medical Center for his hemodialysis session. Due to his current
situation, we are truly in need of financial assistance for his 3x a week session and his daily
medicines intake. Attached herewith is the medical certificate, medical abstract and quotation
for his sessions.
