CLINICAL INSTRUCTOR: Ma’am April Fernandez

1. OBJECTIVES
A. GENERAL: To learn and gain my knowledge about the problems with the
passenger, problems with fetal position, presentation or size and problems
with the passage.
B. SPECIFIC: To know more about the different problems with the passenger,
fetal position, presentation or size and the passage.

2. OBSERVATION
A. SELF: I may not sharing my ideas but I’ve listening and take down noting
while our instructor discussing.
B. GROUPMATES: They are actively responding to our instructor when she had
questions.
C. CLINICAL INSTRUCTOR: She discussed the lessons well.

3. LEARNINGS:
A. LEARNING INSIGHTS: I learned the different problems with the passenger,
the passenger is the fetus or baby. The size of the maternal pelvis, when the
head of the baby is bigger than the pelvis of the mother can cause difficulty of
baby to come out, Umbilical cord prolapse when the umbilical cord of the
baby come out first than the baby and this can be the cause that the baby
can’t be come out. Multiple gestation can cause unequal nutrition. The rule of
umbilical cord is to supply nutrients and oxygen. If the baby is distress it can
cause fetal hypoxia and fetal anoxia and the management of this is to always
monitor fetal heart rate. Oligohydramnios when the amniotic fluid is less. The
multiple gestation high risk is unequal nutrition. The mother can also suffer in
the multiple gestation. Gestational diabetes happens when the woman is
pregnant and can also be when she’s done birthing. When it has pre-
eclampsia, it can also cause proteinuria. Breech presentation management is
through cesarian section delivery. Hydrocephalus pertains water and head of
the baby. If you have CPD as early 34-37 weeks then health care provider
can know what type of delivery you have to encounter. One complication of
forceps delivery is compress or damage of fetal nerves of the baby. It is to
perform amniotomy if the membranes did not rupture. If the bladder of the
mother had fluid, then it can cause blocking. Vacuum extraction complication
is deforming of the head of the baby. It has many complications when vacuum
extraction didn’t perform well. First stool of the baby is meconium. Meconium
staining is usually suctioning.

C. RECOMMENDATION: My recommendation is to be more patience to us.
D. COMMENTS & SUGGESTION: I suggest to be more good instructor to us,
be patient in teaching us and discuss the topic that we can understand clearly
and can gain our knowledge too.
