INFORMED CONSENT FORM

Project Title: Perspectives and Challenges of College Students in Online Class During
Pandemic.

Purpose and conduct of study

This is a descriptive study focusing on the perspectives of undergraduate students and


the challenges they face with regard to online learning. Data gathered in the study
could be used in revealing the perspectives of college students and the challenges they
face with regard to online learning.

Participants’ information (nature and extent of involvement)

Once you decide to participate in this research study, you will be asked by the
researcher for your time to fill up the profile form and the research instrument. You will
be asked for your personal information on the profile form such as age, gender and
grade level. While on the research instrument, you will be asked to fill up the check list
of opportunities and threats statements about the online learning set up experiences.
The data and information will be collected and to be used by the researcher

Risk and inconviniences

Aside from the time commitment required to engage in the study, there are no major
risks associated with it. You are not forced to respond to any questions in the study
instrument that you consider are sensitive, private, or uncomfortable.

No other information will be gathered, and any relevant data will be utilized purely for
the purposes of the study. On average, it will take you 5 to 15 minutes to answer and
complete the research instrument.

Possible benefits for the participants

Participating in the study provides no direct benefit. The study's findings, on the other
hand, have the potential to yield significant societal advantages. Your honest and
sincere response to this data collection can assist to describe the college students'
perspectives and challenges in online class during the epidemic.

Provisions for injury or related illnesses

Because this study will just be a survey, the possibility of injury or disease as a result of
your participation is quite minimal.

Voluntariness of participation

Participating in this study and giving personal information is voluntary. Participants can
withdraw their participation in this research study anytime.

Confidentiality and Data Management

The researcher will ensure that your identify is kept private. The information you
provide will be kept private and treated with strict confidentiality. The data files will be
kept in secure and protected locations that only the researcher will have access to. You
will not be individually identified in any survey reports. The data that will be maintained
will solely apply to your responses to the surveys, and no data that could identify
you. Any reports resulting from this study will not identify you in any manner.
