V. SHORT NARRATIVE REPORT/PROCEEDINGS:

The School Learning Action Cell (SLAC) started at 9:30 AM at the library. The
resource speaker of the topic “Updates on classroom monitoring using CID
Forms”. The resource speaker discusses on what form to be prepare and
accomplish, during classroom structuring and classroom observation, etc. She
stressed to follow CID Form 11B for classroom structuring, in preparation for
classroom evaluation, CID FORM NO. 5 for CLASSROOM MANAGEMENT/Clean,
Conducive, Secure, & Energy-Saving Learning Environment, CID FORM NO. 7 for
Monitoring Checklist for Teacher/Class Observation, CID FORM NO. 8 for RRE-
Class Observation, CID FORM NO. 4 for CLASSROOM MANAGEMENT, CID FORM NO. 3B
for DLL, and CID FORM NO. 1 for BOW or LCD.

VI. ISSUES/CONCERNS RAISED:

Teachers are truly guided in using CID forms in their teaching career.

VII. RESOLUTIONS:

Continuous use of CID forms.
