AFFIDAVIT OF CONSENT

I, HELEN VIRGINIO RIÑOS, of legal age, Filipino, and resident of

No. 25 Diamond St., Emerald Village, Malanday, Marikina City, after
having been duly sworn to in accordance with law, do hereby depose
and state:

1. That I am the owner of a house and lot located at No. 25

Diamond St., Marikina City, being the widow of my deceased
husband, CRISANTO M. RIÑOS;

2. That my grandson JOSE CRISANTO CO is applying for the

installation of separate Meralco Electric Meter to the subject
property for his own electric consumption;

3. That I hereby give my consent to the said installation of a

separate Meralco Electric Meter in favor of my grandson JOSE
CRISANTO CO;

4. That I am executing this Affidavit to attest to the truthfulness of

the foregoing facts and for whatever purpose this may serve.
