AFFIDAVIT OF ACCIDENT

I, MELANIO B. CALIDO, of legal age, Filipino citizen, married

with postal and residence address at No. B 9 L 54 P 6 ACM
Woodstock Subd., Alapan II-A, Imus City, Cavite, after having been
duly sworn in accordance with the law, depose and state:

1. That, my wife, TERESITA CALIDO, is the registered

subscriber of the Meralco Electric Utility Services with
Meter No. 33TZN83054 and Billing Statement No.:
4132065116586;

2. That, the above-stated electric meter device currently

installed on the erected Meralco concrete post three (3)
houses distance from the abovementioned residence
address;

3. That, on June 26, 2022, at about 4:30 P.M., I found out the
aforesaid device has a visible broken portion on the covering
glass which might affect the security and integrity of the
above-said device;

4. That, neither people nor Closed-Circuit Television (CCTV)

witnessed or captured the incident;

5. That, during the same day I immediately reported the said

incident to the MERALCO Bacoor Business Center located
at General Emilio Aguinaldo Bacoor City;

6. That, I am executing this Affidavit to attest to the

truthfulness of the foregoing facts, and likewise I would like
to request an immediate replacement for the damaged
MERALCO electric meter device.
