I, Abigail K. Bulan, Filipino citizen, of legal age, Single, and a resident of 359 Altura Ext. Sta.
Mesa, Manila, after having duly sworn to in accordance with law, hereby depose and say:

1. That I am the Administrator of TRADERS INDUSTRIAL SUPPLY CO., INC., a duly

registered corporation under Philippine laws, with office address at 24th Floor Trident
Tower 312 Sen. Gil Puyat Ave., Makati City 1209 Philippines.

2. The first attempt going to the house of Mr. Mark Andrei C. Angeles the owner of MSV
Construction & Supplies the home address located at 26 Aquila Dr. Villa Orion
Subdivision Fairview Quezon City was February 13, 2024, at 11:05am. To served, the
Summons Notice of Hearing M-MKT-23-06561-SMC to Mr. Mark Andrei C. Angeles.

3. But the personal driver of Mr. Mark Andrei C. Angeles named Mr.
Jose Tandoc Mesina said that Mr. Mark Andrei C. Angeles already left the house in
Fairview Quezon City for almost more than 2 years because of family issues with his
wife but Mr. Jose Tandoc Mesina the personal driver of Mr. Angeles for almost 10
years in the service Mr. Jose Tandoc Mesina he is the Authorized / Representative of Mr.
Angeles to receive the copy of Summons Notice of Hearing with date received of Mr.
Jose Mesina Tandoc February 13, 2024 at 11:20am. In front of the house of Mr. Angeles
in Orion Subdivision Fairview Quezon City, Mr. Tandoc present the Driver’s License Id
No. C11-90-047801 of Mr. Tandoc.

4. Mr. Jose Mesina Tandoc said he will inform Mr. Mark Andrei C. Angeles regarding the
Summons Notice of Hearing. It will be held on March 7,2024, at 8:30am at the court of
Branch 128, MeTC Hall of Justice, Makati City.
