    Respected Madam,

    Reference to subject matter refers;

    1- it is to bring into your kind knowledge that I applied for leaves from 1st Aug, 2022 to 18-
    09-2022 for construction of my house in my hometown (Shahdadpur) which was damaged
    during the recent floods in Sindh. These leaves were duly approved.

    2- During the construction work I got injured and suffered from a severe backbone injury
    and associated lumber pain for which I was admitted to the hospital from 10-09-2022 to
    ______, At the time of discharge doctor advised me regular physiotherapy and 5-month's bed
    rest.

    3- Therefore; it is requested to accept my apologies for not duly informing about my


    absences and withdraw the explanation letter for taking any disciplinary action , on
    humanitarian basis/grounds, please.
