VOLUNTEER TEACHER AGREEMENT FORM

This agreement is hereby made between ROSE DELA PEÑA NATIVIDAD, (hereafter referred to as
the volunteer teacher) and JESSA C. GUBAT, Brigada Pagbasa Chairman of TIERRA NEVADA
NATIONAL HIGH SCHOOL Tierra Nevada, Tinambac, Camarines Sur (hereafter referred to as the
representative).

The volunteer teacher hereby agrees to donate his/her time, effort, and services to TIERRA NEVADA
NATIONAL HIGH SCHOOL in a volunteer capacity. The volunteer teacher understands that no
compensation of any kind will be given in exchange for these services and that no employer-employee
shall arise therefrom.

The volunteer teacher will participate in a volunteer work in a minimum of _____months from the
execution of this contract, working a minimum of _________ hours per week. The volunteer teacher
further agrees to track his/her hours through the teacher's logbook and submit them to the
representative at the end of each month.

The volunteer teacher agrees to undergo all necessary capacity development activities to perform
his/her duties. The volunteer teacher will be engaged primarily to assist regular teachers in facilitating
learning. The volunteer teacher may be asked to perform other duties related to Brigada Pagbasa
within the contract period at any time.

The volunteer teacher agrees to abide by the rules, regulations, orders, and requests as provided in
DepEd orders, Memoranda, and other issuances as well as those promulgated by the Civil Service
Commission. Failure to do so may result in the termination of this agreement and thus no certificate will
be issued as proof of service.

The volunteer teacher of the Brigada Pagbasa representative may terminate this contract early at any
time for any reason without penalty.

MEMORANDUM OF AGREEMENT

The undersigned parties referred to as volunteers hereby agree to donate her time, effort and services
to TIERRA NEVADA NATIONAL HIGH SCHOOL in a volunteer capacity according to the terms and
conditions written in the Volunteer Agreement Form.

Greetings of Peace!

One of the goals of education is to let the students experience active, hands-on activities for practical expertise to
attain quality learning. With this, the Tierra Nevada National High School offers Bread and Pastry Production
subject to Junior High School students from grade 9 to 10 (specialization) and Senior High School students as
their elective subject. However, one of the problems of the school is the lack of tools and equipment to be used
during the practical activities of the students. We believe that students learn most by actually performing the task.
In addition, life-long learning will take place when the students can manipulate tools and equipment needed for
their
practical activities.

The school would like to ask for financial assistance from the Office of the Municipal Mayor of Tinambac
amounting to nineteen thousand four hundred pesos only (Php 19, 400.00) to purchase the identified
tools/equipment for the laboratory room of bread and pastry production of Tierra Nevada National High School.

Hoping for your usual support and consideration. Your kindness will be highly appreciated and will be cherished
forever.

With the full implementation of face-to-face classes set this coming November classrooms are expected to be
filled with the usual number of students. Because of this, teachers are expected to give their space inside the room
to accommodate the large number of students. Since the school do not have a vacant space that can serve as a
faculty room, a make-shift room is the best option.

This room will serve as area for the teachers during their vacant period wherein they can perform others tasks
such as checking the students' output and preparing instructional materials.

The school would like to ask for financial assistance from the Office of the Municipal Mayor of Tinambac
amounting to twenty-eight thousand four hundred fifty-five pesos only (Php 28, 455.00) for the construction
of this project.
