ACCOMPLISHMENT REPORT AS FEEDING COORDINATOR

School Year 2021-2022

Enumerated hereunder are the accomplishments of the undersigned as Feeding

Coordinator of Gaya-Gaya Elementary School.

1. Represented the school meeting organized by the authorized officials regarding the
programs and projects of the Divisions with a connection in the implementation and
dissemination of program of Feeding Program and Milk-Feeding.
2. Represented the school in the meeting for reporting of accomplishment report as
Feeding coordinator.
3. Disseminate information among teachers and the implementation of programs and
activities of the health during our webinar session.
4. Update and monitor the list of feeding and Milk feeding Beneficiaries.
5. Upload list to the drive link.
6. Monitored the delivery of food packs and milk
7. Assists and helping the distribution of food packs and milk to the beneficiaries.
8. Prepared the list and collect data of the beneficiaries.
9. Complied the documents for liquidations report.
10. Monitor and helped in the distribution of the second trach of milk-feeding
11. Monitoring of Feeding thru messenger to their corresponding teacher adviser.
12. Coordinate with Division Feeding Coordinator for the query and updates.
