DESIGNATION 0F FREEDOM OF INFORMATION (FOI) RECEIVING OFFICERS AND

DECISION-MAKERS

1. Per Unnumbered Regional Memorandum from RD Nicolas T. Capulong,

Ph.D., CESO Ill, Director IV, Regional Director dated August 12, 2021 and pursuant
to DepEd Order No.19, s. 2021 entitled ``Rez/isecz Depct7tme71€ a/EcZLtcQct.07l Peapze's
Freedom of informc[fion Mcunual cmd Impze:merLting Detoils», all schooLs shah
designate their FOI Receiving Officers (FOI Ros), FOI Decision Makers (FOI DMs)
and AL1:ernate Decision Makers (FOE ADMs).

2. The same DepEd order mandates all school heads to designate the FOI RO
from the Guidance Counselor's Office or Registrar's Office. Likewise, school hea.ds
shall also recommend an FOI DM as well as an alternate FOI DM who shall act as
FOI DM Officer-In-Charge in the event that the designated FOI DM is unable to
fulfill the role for their respective schools, both with ranks of not lower than Head
Teacher, whose designation shall be approved by the DepEd Secretary.

3. AIl schools are directed to submit on or before August 26, 2021 the names
of their designated FOI ROs, FOI DMs, and alternate FOI DMs through this link

4. Should there be changes on the list of F`OI ROs, FOI DMs, and alternate FOI
DMs, please inform and send updated list to the Schools Division Office - Records
Unit.
