- Menarche 13 years-old, interval regular 30 days, duration 5-7 days, amount 6 pads/day, no
dysmenorrhea
- LMP : 18/12/65
- PMP : uncertain
- Last SI 1 month PTA - no dyspareunia
- No history of abortion
- Contraception : TR
- Last Pap smear : 3 years PTA (normal)
- Previous gynecologic examination : none

Physical examination:
Vital sign BP 147/66 mmHg, HR 102 bpm, RR 16/min, BT 36.6 °C
BW 69 kg, Height 155 cm, BMI 28.7 kg/m^2
GA: A thai female, good consciousness, well-cooperate
HEENT: mild pale conjuctiva, anicteric sclera, no palpable neck mass
- Heart: normal s1s2, no murmur
- Lungs: clear, equal breath sound both lungs
- Abdomen: soft, not tender, no guarding, no palpable mass
- Extremities: no edema
Pelvic examination
MIUB : normal
Vagina : normal mucosa
Cervix : os closed, no lesion , nabotian cyst
Uterus : N/S, not tender
Adnexa : no mass
Culdesac : no bulging
