Instructions:

1. Copy Rating Criteria from the Customer Interview / Vendor Evaluation

Form into rows 11 - 20. (They are automatically copied to lower data
sets) Rating Form

2. Establish Weight Factors for each Rating criterion. Weight Factors

can be whole numbers from 1 to 5. The higher the Weight Factor, the
more influence that criterion will have in the ratings.
3. Enter Vendor Names in yellow boxes

4. Have the same Project Team member(s), Technical staff member(s)

and User(s) rate each Vendor using the Customer Interview / Vendor
Evaluation Form.
5. Enter data from Customer Interview / Vendor Evaluation Forms onto
this worksheet.
