SHE MOMENT
Y Phiri encouraged everyone to keep well hydrated in the hot season, especially between11am and 2pm when the sun is
hottest, to take breaks if working in direct sunlight and carry water bottles. He also cautioned the house to be alert about
critters that may enter the home and ensure windows and doors are closed at night.
3.0 OPENING REMARKS
The Chairman began with the Zero Harm Affirmation. He informed the house that the Department is behind schedule
with the document review, which is a cause for concern as there are documents which urgently need revision so that
they are up to standard. He observed that there is room for improvement with regards to the Department’s leading and
lagging indicators. He also noted that there will be a discussion on blind spots in various sections later in the meeting,
highlighting that all practices should be documented so that solutions for the blind spots are proffered. Finally, he urged
supervisors to engage their teams to ensure that those who were not present for the meeting were also updated.

4.0 PASSING OF MINUTES OF THE PREVIOUS MEETING


The minutes of the previous meeting were proposed by I Mukamuri and seconded by T Chinyama and were adopted as
a true record of what transpired in the previous meeting pursuant to corrections being made.

6.0 SHE STEERING MEETING FEEDBACK
6.1  Meeting not yet held for the month.
 SHE Liason meeting was held.
 SHE performance minewide has been good; 3 million fatality free shifts have been achieved.
 Guard against complacency
 SHE moment was “Working from heights”; it will be circulated as a tailgate
 One MTC was recorded in October and one LTI in July
 Put a stop to accidents so that we end the year well

7.0 SELECTED SHE TOPIC(S) FOR DISCUSSION- Blind Spots (M Gumbie)


 Topic on what blind spots are was conducted in daily tailgate.
 Insufficient PPE can cause injury
 Standby duties conducted as a shift cause fatigue during normal working hours.
 Blaming others
 Labour shortage is contributing to productive but undocumented practises.
 Stores is not centralised so labour has to be spread across multiple warehouses.
 Ineffective communication
