INCIDENT REPORT

Before:
On 25th December 2022 at 14.0.6 AM, Senayan City inform BBP about our temperature transmitter
TT-13 emit clunking sound because vibration (see Fig. 1).

The vibrations seemed destructive and, since they are running a test for the chiller that just
finished maintained, so they asked BBP to investigate it immediately.

Action:
In the same day, BBP responded and acted immediately, to check the condition and then
dismantled the thermowell because BBP suspect it was the root cause.

Identification:
1. BBP found that the thermowell was in loose condition, while it should be the opposite
(see Fig. 2)
2. BBP then tried to pull the U-length out from the adaptor, then BBP found that the U-
length actually was fractured (see Fig. 3)
3. After conducting an investigation with the team, and based on previous incident that had
occurred on Singapore sites, we conclude that improper design was the cause; the U-
length should not exceed half of diameter of the existing pipe.

Corrective action:
1. BBP with Senayan City team installed temporary cable to replace the broken one – See Fig
7 and re-route cable for busbar – See Fig 8.
2. BBP with vendor (Voltech) re-checked all connection, on 18 th November 2022 at 10 PM –
See Fig 9. Then we did test run for all cooling tower on 19 th November 2022 at 3 AM – See
Attached 1. Last, we did standby until operational time started for cooling tower and all
cooling tower fans are working properly.
