1 PURPOSE

The purpose is to provide guidance for establishing a system of safe travel throughout all
Nevada Gold Mines (NGM) sites. Each division shall implement a Surface Traffic Management
Plan, detailing safe practices and risk control measures to be put in place to protect workers,
equipment, and facilities.

2 SCOPE

This plan applies to all Barrick operating, project, operated joint ventures, and exploration sites
and is relevant to Health, Safety, Security, and Environmental Practices. It shall apply to all
departments, employees, contractors, and visitors. This standard applies to all surface
operations, including underground surface operations.

3 DEFINITIONS AND ACRONYMS

3.1 Definitions
50/20 Rule - When approaching within 50 yards of Heavy Equipment, positive 2-way
communications will be made with the operator letting them know you are in their work area.

• If approaching within 20 yards of Heavy Equipment, positive 2-way communications will


be made with the operator letting them know you would like to enter the 20-yard area.
Before entering, the Heavy Equipment operator will properly park and exit the cab before
entering this zone.

Active Work Areas - Any location where active mining, processing, maintenance operations or
support functions are conducted.

Blind Side -The side of the equipment opposite of the operator’s cab.

Exclusion Zone - An area into which unauthorized personnel and/or equipment are not allowed
to enter.

• Marking these zones can be accomplished by using the materials defined in the
“Barricade” and “Barrier” definitions in the Equipment Safeguards and Protective Devices
Procedure.

Left Hand Traffic - All bi-directional traffic is to keep to the left-hand side of the road.

Mobile Equipment

• Off Road Equipment - Sport Utility Vehicles (SUVs), or All-Terrain Vehicles (ATVs) such
as four wheelers and snowmobiles.
• Light Duty Vehicle (LDV) - Any small vehicle specifically designed for transportation of
people or freight, i.e. pickup trucks, vans, ATVs, or SUVs. Typically, ¾ ton or less in
weight.
• Light Equipment (LE) - Any large vehicle specifically designed for transportation of
people or freight, i.e. busses or semi-type over the road hauling equipment. Typically,
greater than ¾ ton in weight.

• Heavy Equipment (HE) - Any vehicle or piece of Mobile Equipment specifically designed
for executing construction, mining or demolition tasks, and most frequently involving
earthwork operations. This includes Service type trucks.

Positive Communications - Radio communications or face to face communications that are


associated with two-way communication. This requires confirmation of “unit number” to “unit
number”. For example: Truck 36 to Dozer 15 am I clear to pass you on your right side? Dozer
15 to Truck 36 you are clear to pass me on my right side.

• Light signals are not a form of positive communication.


• Active road maintenance needs positive two-way communications.

Queue (Q) Area - A parking area designed for heavy Mobile Equipment (Permanent or
Temporary). The preferred Queue Area design is first movement forward and no reverse
parking.

Reverse Parking - Light duty vehicles will be required to back into a parking location, unless
otherwise posted.

Shift Change Parking Areas - A temporary parking area/ditch constructed near active pit
mining area to secure heavy Mobile Equipment. If able, first movement forward design shall be
implemented.

3.2 Acronyms
ATV - All-Terrain Vehicle

4 CONTENT

4.1 Employee Personal Vehicles


Sites that have employee parking lots shall require employees who choose to drive a personal
vehicle to work to park at the designated parking lots.

Absolutely no personal vehicles are allowed onsite without prior authorization from divisional
General Supervisor, Superintendent, or Manager.

4.2 Contractors
All contract employees shall adhere to the site access requirements set forth in the Business
Partner Management Procedure.

4.3 All Mining Operations


Where designated, all personnel operating any Mobile Equipment shall have some form of an
Open Pit Driver’s License (typically, a current 5000-23).

4.4 Mobile Equipment Start-Up


Prior to starting or moving a piece of equipment, the following warning signals shall be used:

4.4.1 Light Duty Vehicles/Light Equipment


• Starting a piece of equipment – ONE horn blast followed by an approximate 3 second
delay prior to engine start.
• Moving a piece of equipment forward – TWO horn blasts followed by an approximate 3
second delay prior to movement.
• Moving a piece of equipment in reverse – THREE horn blasts followed by an approximate
3 second delay prior to movement.

4.4.2 Heavy Equipment


• Starting a piece of equipment – ONE horn blast followed by an approximate 5 second
delay prior to engine start.
• Moving a piece of equipment forward – TWO horn blasts followed by an approximate 5
second delay prior to movement.
• Moving a piece of equipment in reverse – THREE horn blasts followed by an approximate
5 second delay prior to movement.

4.5 Parking Procedures


Reverse Parking - On the mine site, light duty vehicles will be required to back into a parking
location unless otherwise posted. Pull-through parking areas are acceptable as to provide “First
Movement Forward”.

4.5.1 Surface Mining Operations


• The transmission controls shall be placed in a parked position, when applicable.
• The parking brake, if so equipped, shall be set.
• Mobile Equipment shall back into designated parking areas, parking ditches, or tires
chocked with 2 chock blocks.

­ First movement should always be in forward motion to avoid having to back into high
traffic areas. Pull through parking design is acceptable.
­ If equipped, all implements shall be lowered to lowest position, mechanically secured,
or resting on the parking surface.

• All equipment, including tracked equipment, when parked and left unattended on a grade
shall be blocked against motion by using appropriate chock blocks or turned into a bank

or berm.

4.5.2 Queue Area Parking Controls (Permanent)


• First movement forward; options include:

­ Pull through design from operators left to right (preferred), and / or


­ Back into parking ditches from operators left to right.

• Secure Mobile Equipment from unwanted movement.


• Adequate separation controls (limiting blind spot hazards) for on and off boarding of
personnel and maintenance procedures.
• Illumination for low visibility.
• Supply Conex (optional).
• Signage to direct traffic for all Mobile Equipment.
• Environmental controls (spill kit).
• Portable restrooms shall be placed in designated areas with surrounding barricade sized
to the largest equipment working in that area.
• Adequate drainage design.
• Any deviation of preferred Queue area design shall follow Management of Change
(MOC) procedure.

4.5.3 Shift Change Parking Area (Temporary)


• First movement forward; options include:

­ Pull through design from operators left to right (preferred), and / or


­ Back into parking ditches from operators left to right.

• Adequate parking controls for haul trucks (i.e. parking ditches).


• Illumination for low visibility.

4.6 Pedestrian Traffic/Ground Control


4.6.1 Surface Mining Operations:
• Pedestrians shall be on designated walkways or in designated congregation areas when
available.
• Work areas that consist of foot traffic shall be delineated in a manner to separate the
work area from the traffic flow.
• Pedestrian foot traffic is not allowed on active haul roads unless additional controls are
put in place for pedestrian crossing.
• Unless posted otherwise, anyone working outside of equipment shall abide by the NGM
Personal Protective Equipment (PPE) Technical Procedure.
• If there is a need to approach equipment that is being operated, there shall be Positive
Communication made and the 50/20 Rule will be utilized (refer to section 10 below).

4.7 50/20 Rule


4.7.1 Light Duty and Light Equipment Entering Work Areas of Heavy Equipment (HE)
• When approaching HE (excluding Drills), you must make positive 2-way communication
with the operator advising them that you wish to enter within a 50-yard radius of the
machine. Permission from the HE operator via positive two-way radio communication

must be received prior to entering this zone.


• Do not enter the 50-yard zone until positive 2-way communication has been made.
• If you intend to park in the 50-yard radius of the HE, the LV/LDV must be parked in a
safe area or within a segregation berm, outside the 20-yard radius of the machine.
• When your intent is to interact with the HE and you will be within 20 yards of the machine,
the following must occur:

­ Contact HE operator via positive two-way radio communication advising them of your
intent to enter within 20 yards of the Heavy Equipment;
­ The HE operator must provide positive two-way radio communication and lower all
ground engaging tools;
­ The HE operator is to exit the cab of the equipment;
­ The LV/LDV may now enter the 20-yard radius of the HE;
­ When the HE operator has visually confirmed the LV/LDV has left the 20-yard zone,
the operator may re-enter the HE cab and resume operations; and
­ When exiting the 50-yard zone, positive two-way radio communication must be made
with the HE operator, notifying them you are clear of the area.

4.8 Entering the Working Area of Drills


When approaching a drill, you must make positive contact with the drill operator prior to entering
the coned area. Permission from the Drill operator via positive two-way radio communication
must be received prior to entering the coned / bermed area.

If you intend to park in the coned / bermed area of the Drill, the LV/MDV must be parked in a
safe area or within a segregation berm, outside the one row zone.

When your intent is to interact with the Drill and you will be within the one row zone, the following
must occur:

• Contact Drill operator via positive two-way communication advising them of your intent
to enter within one row of the Drill;
• The Drill operator must provide positive two-way radio communication and cease drilling
operations;
• The Drill operator is to exit the cab of the Drill;
• You may now enter the one row zone of the Drill;
• When the Drill operator has visually confirmed the LV/LDV has left the one row zone, the
operator may re-enter the Drill cab and resume operations; and
• When exiting the coned area, Positive Communication must be made with the Drill
operator.

4.9 Signage and Delineation


4.9.1 Surface Mining and Underground Mining Operations
• Signage shall be placed in an obvious location, in a manner that is readily visible from all
mobile equipment.
• All signage shall be on a preventative maintenance schedule for cleaning and/or
replacement.
• Appropriate signage shall be placed at all approaches to Active Work Areas where

hazards could exist for Mobile Equipment.

Sites to determine if, and where delineation guide posts may be installed on all haulage and
travel ways to include factors such as following distances, corners and curves.

4.10 Communication Protocols


4.10.1 All Mining Operations
• All Mobile Equipment shall have 2-way positive communication when interacting with
mobile mining equipment. All mobile equipment shall be equipped with 2-way radios or
operator must have a portable 2-way radio.
• Mobile Equipment operators shall operate on the designated radio channel for the area
in which they are traveling in.
• Emergency communication shall follow the guidelines in the NGM Event Management
Technical Procedure.

4.11 Use of Electronic Devices


• The use of electronic devices will not be utilized in active mining areas unless in a designated
area, pulled off the road in a safe location, or have other controls in place to ensure the safety
of the personnel.

4.12 Emergency Procedures


4.12.1 All Mining Operations
• All mobile equipment shall have emergency shutdown procedures.
• All personnel will be trained on emergency procedures and muster areas.
• In an emergency the operator is to utilize the “MAYDAY” call over the radio and follow
mayday procedure.

4.13 Traffic Rules and Procedures


4.13.1 Surface Mining Operations
• Traffic patterns shall consist of left-hand traffic.
• Left hand traffic shall be denoted with signs posted at all right/left hand
intersections/crossovers, and any location the lane of travel changes.
• Heavy equipment and light duty/light equipment shall be segregated wherever possible.
• All mobile equipment shall operate with headlights on at all times.
• All light duty vehicles traveling in active mining areas shall be equipped with a functioning
strobe light, reflective material on the rear and both sides of the vehicle and a buggy
whip.
• All contractor vehicles shall be equipped the same along with the company name
displayed on both sides of the vehicle.
• Roads blocked or gated for blasting, emergencies or any other situations will be
designated by signage, personnel, or mobile equipment turned sideways in the road.
• Roadblocks shall not be passed without authorization.
• All overhead infrastructure shall be labeled indicating the height.
• All exclusion zones must be properly marked, labeled, and/or barricaded.

4.14 Right of Way


4.14.1 All Mining Operations
• Pedestrians shall always be granted the right of way unless mobile equipment is
responding to a mine emergency.

4.14.2 Surface Mining Operations


• Surface right-of- way shall be granted in the following order:

1. Ambulance or other mobile equipment responding to an emergency.


2. Pedestrians.
3. Explosives transport.
4. Active road maintenance equipment. (Positive 2-way communications required)
5. Loaded haulage equipment (unless signage dictates otherwise).
6. Unloaded haulage equipment (unless signage dictates otherwise).
7. Support equipment.
8. Bin loaders.
9. Light duty/light equipment.

• Anywhere haul roads merge, right-of-way shall be given to trucks on the left due to the
blind side merging with traffic, unless signage dictates otherwise.
• Anywhere active road maintenance is in place all equipment need to make two-way
positive communications.

4.15 Speed Limits


4.15.1 All Mining Operations
• Operators shall obey all posted speed limits.
• Operators shall drive in a manner that is safe for the condition of the road.

4.15.2 Surface Mining Operations


• Unless otherwise posted, the maximum speed limits within active mining areas or heavy
equipment areas that are designated with left hand traffic shall be:

­ Haul roads – 45 MPH;


­ Dumping and loading areas – 15 MPH; and
­ Shops, staging areas, laydowns, processing facilities, fuel bays – 10 MPH.

4.16 Passing/Overtaking Mobile Equipment


4.16.1 Surface Mining Operations
• Passing and overtaking mobile equipment is only permitted when positive radio
communication has been established.

4.17 Following Distances


4.17.1 Surface Mining Operations
• A minimum following distance of 200-ft shall be maintained whenever following heavy
mobile equipment.
• The minimum following distance for light duty and light equipment shall be a minimum
distance of 50 feet or the use of the 3-second rule.
• Following distances shall increase if adverse weather conditions exist.

4.18 Adverse Weather Procedures


4.18.1 All Mining Operations
• Each Nevada Gold Mines site shall provide detailed guidelines for mitigating hazards that
arise due to inclement weather.

4.19 Road Design


4.19.1 All Mining Operations
• The width of a haul road is determined by the width of the widest piece of equipment that
will travel it (reference NGM Haul Road Design Manual).
• This manual shall include but is not limited to intersection layout, berm
design/construction, water management, and dust control.
• When practicable, all intersections need to be at 90-degree angles and no more than 4-
way crossings. Other considerations may be applied based on a formal risk assessment
(i.e. roundabouts).

4.20 Incident/Accident Reporting


• All incidents/accidents regarding mobile equipment interaction shall be reported and
investigated as per the Event Reporting and Investigation Procedure.

4.21 Risk Management and Assessment


• There shall be a Fatigue Management Plan / Standard that considers cumulative hours
of service, shift length, and time in seat.
• All mobile equipment shall be evaluated through a risk management process.
• Any proposed variance to this standard shall be put through the risk management and
MOC process.
