1. Ref, .Memorandum dated March 28,2022, from your office directing the
undersigned to explain in writing why I am absent during the conduct of morning
formation last March 28,2022..

2. Please be informed that I was on my way that morning to attend the morning
formation but while driving along Mc Arthur Hi-way particularly at San Sebastian
Village a vehicular accident transpired between a single motor cycle and a
passenger jeepney. I decided to temporarily assist the traffic while waiting for the
duty investigator and when I arrived at this station the morning formation was
already finished. Said accident was recorded in our police blotter and P/CPL
Armando dela Vega could attest on the incident, because I properly turned over to
him the incident.

3. It is not my intention not to attend the morning formation hoping for your kind
consideration.
