1. GENERAL
1.1. PURPOSE
Risk Assessment conducted at Apartement Gardenia @Bogor on 07 May 2019 on Jl. K S Tubun
No.102, Cibuluh, North Bogor, Bogor, West Java. Within the framework of insurance coverage and
to know the latest conditions on the subject of coverage and risks that are in the object.
And look at aspects of general construction and protection facilities against fire, blasting and other
risks.
This information is used to evaluate existing prevention and control measures, and also suggests
improving and reducing overall hazards.
During conducting a location survey, we met with several people representing the Insured, as
follows:

1.2. DISTRIBUTION
The report is used by the Insurer, the relevant Branch Office and Some Reinsurance Companies
and or Reinsurance Brokers.
Recommendations for Rising Risks are submitted to the Insured by the relevant Branch Office.
2. OCCUPANCY
PT. Duta Senawijaya Mandiri is part of the Takke Group engaged in property business. Takke Group built
a Gardenia Bogor apartment on an area of 1.3 ha and there are two towers. Each tower has a height of
25 floors which summarizes 1,539 units of various types. The number of units is divided into 465 units of
Raflesia tower and 1,074 units of Bougenvile tower.
Various facilities are provided at the Gardenia Bogor apartment, including a swimming pool, jogging
track, playground, fitness center, sky garden, 24-hour security, and commercial area. Here also will be
built a 1,000 m2 commercial area and lifestyle center to meet the needs of residents.
The project area is 88,683 M2. This apartement has 2 towers building. They are Bougenville (U shape)
and Raflesia (L shape).
There is 3-meter-high zinc fence that surrounds the project area as a barrier to the outside area
and the entrance door on the site is guarded by security.
From the basis of the boundary factors and the properties located around the location, the
exposure of the surrounding property can be considered Low to Medium.
Based on the existing layout, the building can be included in the category as 1 Fire Zone because
the distance between buildings is unadequate.
between the towers there is an area that is one, namely the commercial area and the basement
area which is the parking area of the vehicle. in that area there is no separate firewall to prevent
the fire from spreading to other buildings
4. PROCESS DESCRIPTION
4.1. OPERATION PRODUCT FLOW
PT Duta Senawijaya Mandiri appoints PT. Pembangunan Perumahan (Persero) Tbk as the main
contractor to build this apartment. This project began in 2014 where the installation of the
foundation using piles was carried out by PT. Sunway Yasa Mandiri. After the installation of the
pile was completed in 2015, the handover of the project was carried out to PT. Pembangunan
Perumahan (Persero) in the same year. This project will be completed in October 2019.
The development of this apartment development has now reached 50% - 60%. in 2017 the
apartment building has been carried out "Topping Off". meanwhile the current stage is the
finishing stage, namely ME (Mechanical Electrical). In finishing work there are also welding
activities
5. HAZARDS ANALYSIS
Layout design This apartment project will have as many as 24 floors of buildings
with additional 2 floors of basement. There are 1527 units of
apartment rooms with studio types and 2 bedrooms types. And there
is also a swimming pool and commercial area that complements the
supporting facilities in the apartment. While the basement will be
used as a vehicle parking lot (193 cars) and a transfomator room,
generator room and hydrant pump room.
Control of process The power cord uses PVC sheathed cables and most of the lighting for
the project using uncovered fluorescent lights.
Material Handling The stock materials that will be used in the construction of apartments
are stored in the project location with a bounding tape. The material
stock is arranged neatly and does not obstruct the evacuation route that
is in the project

Currently the contractor is installing a hydrant system. Most of the hydrant pipes have been
installed and the hydrant box has been installed in the tower apartment. However, the hydrant
pump has not been installed by the contractor yet.
• WaterSupply,
A ground tank with a capacity of 400 m3 equipped by a jockey pump has been prepared by the
contractor but there is no water supply in the ground tank.

6.3. MAINTENANCE
In general, the fire protection system provided at the site is rated medium average compared to
the inherent operational hazards given.
