Dear BM Rommel,

I, HERNOR M. DE ASIS, am writing this letter to authorize Ms. Sahani M. Umpar to obtain my Professional
Regulation Commission (PRC) License ID on my behalf. Ms. Umpar will be visiting your WEXpress,
DHL/Wexpress Bacolod Branch located at Unit 2, Comglassco Bldng, to collect the said ID.W

As I am unable to personally claim my PRC License ID, I am entrusting Ms. Umpar to act on my behalf and
receive the document. She will present a valid identification card and a copy of this authorization letter to
prove her identity and authority to collect the license.

I hereby declare that I am authorizing Ms. Sahani Umpar to obtain my PRC License ID, and I hold her
responsible for the safekeeping and proper handling of the document. I waive any claims or liabilities against
your company, WEXpress, DHL/Wexpress Bacolod Branch, for releasing the said ID to the authorized person.
