AFFIDAVIT OF CONSENT TO TRAVEL ALONE

That I, EMELY CORPUZ-BORRE, of legal age, Filipino, married and

resident of Poblacion 6, Midsayap, Cotabato, after having been duly sworn in
accordance with law, do hereby depose and say:

I had an illegitimate son whom I named as Michael Corpuz, who was born
on December 11, 1993, at Midsayap, Cotabato, and 17 years of age,

That the father of my illegitimate son Michael Corpuz is Miguelito R.

Pajarito, who is presently residing at 1402 Vesta Court, Panorama City, California
(91402) USA;

That I have already agreed that he will be in the custody of his father
Miguelito R. Pajarito who will join him at 14502 Vesta Court, Panorama City,
California (91402), USA ;

That I giving my consent that son Michael Corpuz will travel alone to
California, USA, to join with his father Miguelito Pajarito, because there is no
available person who will accompany him to the United States;

That I am giving my consent without being forced, threatened or

intimidated to allow my son to travel alone to the United States;

That I execute this affidavit to attest to the truth of the foregoing facts
and for whatever legal purpose the same may serve.
