b. Prepare ICS for donated non-consumable items (below 50,000).

● ICS DATE: Apr 27, 2023 (For all donated non-consumable items received in
2022 as per working paper)
● ICS NO: Use the same sequence/number series used for MOOE
For example:
2023-01-001 (MOOE-purchased in January)
2023-03-002 (MOOE purchased in March)
2023-03-003 (MOOE purchased in April)
2023-04-004 (DONATION recorded in April)
2023-04-005 (DONATION recorded in April)
● In the updated ICS form, put a tick mark for “donation” in the upper right corner.

c. Prepare PAR for donated Property, Plant and Equipment (50,000 and above).

● In the updated PAR form, put a tick mark for “donation” in the upper right corner.
 NOTE: For those who have already prepared and issued ICS and PAR of
donations beforehand, you don't have to reproduce a new copy and change the
date. Just write "DONATION" at the upper right hand corner of the form to
identify ICS and PAR of items purchased out of MOOE Fund.

d. Prepare Summary of Donations. Please use specific template for donations for FY 2022
