Re: Notice of Termination of Service Agreement/

Request for Review of Billings/Disputes

Dear Ms. Ygot:

This is to formally notify your good office that we will not be renewing our service agreement with your company
and that we will be vacating the space on the expiration date which will be on November 30, 2021.

As part of our exit clearance process, we appreciate your time to meet with our HR and Admin. Supervisor and
review further our pending for payments. As it has been discussed with you our previous disputes and requests
pertaining to some irregular increases and disputable charges which were not part of the original service
agreement, allow us to further appeal on the following which we pray will be deemed as acceptable:

Exclusion of the Monthly Charge on Connection Amounting to Php15,000.00

Such charge was originally not part of the service agreement and we have requested for validation if such
service may be terminated, given a one-month notice during the 2020 lockdown, may we again request that this
be excluded on our monthly bill.

Exclusion of Fee for Beverages/Refreshments

We appreciate your Management's approval on excluding the charges for Refreshments, amounting to
Php9,000.00/month while we are on strict quarantine. Unfortunately, the charges were still included on the
previous billings. We therefore request for review and if we may please comply with the approved arrangement.

We will also appreciate a feedback on the status of our security deposit amounting to Php176,000.00.

We do hope that we can finalize the charges and billings and consider our appeal and disputes so we may
immediately settle on what is due for your company.
