INTRODUCTION
You are being invited to participate in this study, because you are one of the enrollees of
Manuel S. Enverga University Foundation Candelaria Inc., in the first semester of the school
year 2022-2023 when the full implementation and utilization of Enverga University Student
Information System (EUSIS-Candelaria) started. This consent form provides you with
information to help you make an informed choice. Please read this document carefully. If you
have any questions, they should be answered to your satisfaction before you decide whether to
participate in this research study.
concerns related to the current online enrolment system being utilized by the institution and all
the stakeholders at large. Henceforth, the university will be able to continue its quality service
for the welfare of all its stakeholders.
BENEFITS
This study will be beneficial not just to the university, but also to the students which are primarily
the respondents of the study. Because they will be able to assess the extent of quality that
EUSIS-Candelaria were able to provide. Hence, it might lead to addressing the problems and
issues encountered in using EUSIS-Candelaria. So, the university will be able to determine
which part of the system or procedure does need to be modified or developed to address those
problems, issues and concerns and be an effective service provider during online enrollment
procedure for the following semesters.
ONE’S PARTICIPATION
There will be survey questionnaire entitled “STUDENTS’ ASSESSMENT OF ENVERGA
UNIVERSITY STUDENT INFORMATION SYSTEM (EUSIS – CANDELARIA) AS A DIGITAL
TRANSFORMATION” which will be randomly disseminated to the students regardless of their
age, sex, year level, course and department, this is to make sure that every student will be
given a fair and equal opportunity to answer. The first section is the informed consent form that
is attached in the google form which should be send back or attached with corresponding
esignature of the participants. The second, third and last page is the research questionnaire,
that if in case the student were not able to finish answering the questions, the other answered
question will no longer be accounted, due to large number of participants needed for the study,
the researcher might not be able to ask each of them what might be the reason for not
accomplishing the survey. Because the survey questionnaire might take 10 minutes of
participant’s time, if given a chance to be answered.

RISKS
Participants might be at risk of technological stress or Technostress which according to G.
Bondanini et al., (2020) technostress is a modern disease of adaptation caused by an inability to
adapt with new computer technologies which affects mental health in a manner that may
manifest as a struggle to accept computer technology, or as over-identification with computer
technology. There is little chance for the participants to get this risk, since most of the possible
respondents are Generation Z or the “Gen Z” as well as the Millennials which both have an
advanced knowledge in terms of using computer technology.

Participants understand that the information he/she provides in this study will be used for
research purposes. It will not be used in any manner which would allow identification of his/her
individual responses.
Anonymized research data will be archived at researcher’s personal Google drive thru Google
form and sheet which will be deleted after the accomplishing the study on or before January
2023 in order to make them available to other researchers in line with current data sharing
practices.
