INTRODUCTION
You are being invited to participate in this study, because you are one of the enrollees of
Manuel S. Enverga University Foundation Candelaria Inc., in the first semester of the school
year 2022-2023 when the full implementation and utilization of Enverga University Student
Information System (EUSIS-Candelaria) started. This consent form provides you with
information to help you make an informed choice. Please read this document carefully. If you
have any questions, they should be answered to your satisfaction before you decide whether to
participate in this research study.

STUDY BACKGROUND AND RATIONALE

As the COVID-19 pandemic arose in 2019 which hit the world economy very hard, even the
most stable and rich country, leaving no industry unaffected. This has alarmed the need for a
change in strategizing on how to still conduct businesses to recover and compete with the
challenges given the current restrictions. The same dilemma exists in the educational sector,
with some questioning whether classes will be held in the coming school year and on what
modality the school will be able to teach and conduct other operations. However, restrictions are
present in every transaction, so the institution strategizes on how to still accommodate the
needs of the clients not limited to the learning modality, but also the other services being
provided by the university. To protect the stakeholders (such as the staff, students and even the
parents) of the institution the online enrollment system has been implemented from online
admission, evaluation of subjects, assessment of fees and payment with the use of Enverga
University Student Information System (EUSIS-Candelaria). This digital transformation from
manual process of providing service is what the pandemic brought in MSEUFCI.

On the other side, no matter how good the system is, there is always a chance or margin of
error. And for these instances, the only way to resolve this is to ask for the feedback from the
users (students) of the EUSIS-Candelaria, in order to address the problems, issues and

concerns related to the current online enrolment system being utilized by the institution and all
the stakeholders at large. Henceforth, the university will be able to continue its quality service
for the welfare of all its stakeholders.

BENEFITS
This study will be beneficial not just to the university, but also to the students which are primarily
the respondents of the study. Because they will be able to assess the extent of quality that
EUSIS-Candelaria were able to provide. Hence, it might lead to addressing the problems and
issues encountered in using EUSIS-Candelaria. So, the university will be able to determine
which part of the system or procedure does need to be modified or developed to address those
problems, issues and concerns and be an effective service provider during online enrollment
procedure for the following semesters.

ALTERNATIVES TO THIS STUDY

There is no force in answering the survey questionnaire, though this might be the student's
chance to be heard one way or another but if the student choose not to participate there are
more other students to ask to participate in this study.

ONE’S PARTICIPATION
There will be survey questionnaire entitled “STUDENTS’ ASSESSMENT OF ENVERGA
UNIVERSITY STUDENT INFORMATION SYSTEM (EUSIS – CANDELARIA) AS A DIGITAL
TRANSFORMATION” which will be randomly disseminated to the students regardless of their
age, sex, year level, course and department, this is to make sure that every student will be
given a fair and equal opportunity to answer. The first section is the informed consent form that
is attached in the google form which should be send back or attached with corresponding
esignature of the participants. The second, third and last page is the research questionnaire,
that if in case the student were not able to finish answering the questions, the other answered
question will no longer be accounted, due to large number of participants needed for the study,
the researcher might not be able to ask each of them what might be the reason for not
accomplishing the survey. Because the survey questionnaire might take 10 minutes of
participant’s time, if given a chance to be answered.

RISKS
Participants might be at risk of technological stress or Technostress which according to G.
Bondanini et al., (2020) technostress is a modern disease of adaptation caused by an inability to
adapt with new computer technologies which affects mental health in a manner that may
manifest as a struggle to accept computer technology, or as over-identification with computer
technology. There is little chance for the participants to get this risk, since most of the possible
respondents are Generation Z or the “Gen Z” as well as the Millennials which both have an
advanced knowledge in terms of using computer technology.

PRIVACY AND CONFIDENTIALITY

To protect the participants information especially his/her identity, answering the Personal
Information part under the Name question will be optional however for the other demographic
profile (e.g. age, sex, year level, course & department ) will be required in the survey
questionnaire to have a better conclusion of the study. All of the students’ information and
answers will be treated with utmost confidentiality and will be the responsibility of the
researcher.

OPEN DATA
Participants understand that the information he/she provides in this study will be used for
research purposes. It will not be used in any manner which would allow identification of his/her
individual responses.
Anonymized research data will be archived at researcher’s personal Google drive thru Google
form and sheet which will be deleted after the accomplishing the study on or before January
2023 in order to make them available to other researchers in line with current data sharing
practices.
