This memo serves as a written warning regarding your unpremeditated filing of immediate leave –
a leave on May 13, 2023, Saturday, filed a day before, May 12, 2023, 03:17PM, for the reason of a
Christening event.

There are several oral reminders that leaves shall be filed at least one (1) week to three (3) days before
the said date of leave so that the company will be able to adjust with the schedules, tasks, and
workloads, and employees will have a proper endorsement of the said tasks and workloads.

Despite having the knowledge of that company policy, your actions still disregarded that policy.

This written warning is reminding you of the critical importance of the job assigned to you in this
company. Consequently, you will receive a final verbal counselling and verbal warning for the said
unpremeditated filing of immediate leave.

Should this written warning will not have the impact that the company expects, a five (5) day
suspension and further disciplinary actions should be taken by the management.
