This Monitoring and Evaluation (M&E) Tool on Early Registration, Oplan Balik Eskwela (OBE)
and Advocacy  for SY 2022 - 2023 is to be accomplished by either the school head or OBE or
school information coordinator. Task time is 5-10 minutes. The information that you will
provide will be used for technical assistance provision and possible trainings or orientations
that will lead to strengthened access to education.

A copy of your responses will be sent to your email. Print a copy and submit to the division
monitor/PSDS during his or her visit for validation. 

All information collected in this tool  shall be processed, used and shared by and within SDO
Masbate solely for the purpose of Oplan Balik Eskwela with the right to confidentiality as
mandated by the Data Privacy Act.
