Note: 5. Sub-contractor brings Approved ID Gate Pass Request

1. Sub-contractor fills out the personal data in the ID Gate
Pass Request Form and submit to Project Participants
2. Project Participants submits ID Gate Pass Request Form
to JOSW for approval at least 7 (seven) days on
weekdays before the worker starting to work.
3. JOSW returns Approved ID Gate Pass Request Form to
Project Participant for further process
4. Project Participants distributes Approved ID Gate Pass
Request Form to Sub-contractor for further process with
copy of acknowledgment safety induction
Form to Security Company for Photo Taking and Printing.
6. Security Company submits printed ID Gate Pass to JOSW
for signing
7. JOSW returns Signed ID Gate Pass to Security Company
for Laminating or Plastic Processing (max 7 days) on
weekdays.
8. Security Company distributes Complete ID Gate Pass to
Project Participants or
9. Security Company distributes Complete ID Gate Pass to
Sub-contractor with permit to Project Participants prior
