AFFIDAVIT OF WITNESS
I, JOANNE CUEVAS Y TEPPANG, 21 years old, single, student and a resident of
Block 21 Lot 3 Phase 1 Heroesville, Brgy.Gaya-gaya, San Jose Del Monte City, Bulacan, after
having sworn in accordance with law, do hereby depose and state THAT:

1. I was present and personally saw that accused, Anthon Rivera, committed the crime of
MURDER against the victim Jeremy Gonzaga;
2. On the 20th of May 2022 at around 8 o’clock in the morning, I was having breakfast at
Tito Renz Restaurant along Ipo Road;
3. I saw Mr. Rivera complaining to the waiter, Mr. Gonzaga that there is something wrong
with the food he ordered. He angrily explained that the food served to him is already
spoiled. They have a heated conversation that last for a couple of minutes when suddenly
Mr. Rivera approached and stabbed Mr. Gonzaga on the neck using a bread knife and run
away;
4. Right after the incident, I immediately dial the number of the San Jose Del Monte Police
Station and report what happened to that placed;

I executed this affidavit to attest the truthfulness of the foregoing facts and to support the
filing of Criminal Cases against Anthon Rivera for violations of the crime Murder.
