SWORN STATEMENT

I, JO1 ALMUDZRI D. ASA, of legal age, Filipino, married, and a resident of 1070
Padre Campa St.Brgy 468, Zone 46, Sampaloc Metro Manila, Philippines, Member of the
Bureau of Jail Management and Penology (BJMP) Caloocan City Jail, after having duly sworn to
in acordance with law, do hereby depose and say;

That on January 10, 2022 incident (Riot) happen at Caloocan City Jail. I’m on mandatory
home quarantine due to Covid-19 Positive.

That I cannot give any detail for what happen with regards the incident(Riot) on the said
date.

That, the undersigned executed this affidavit to attest the veracity of the foregoing and
affixed his signature above his name.
