Note 1: All Inspection Requests are to be forwarded to qaqc.kurdistan@dno.no 24 hours in advance unless directly arranged with DNO QA/QC.
Note 2: Contractor QA/QC shall confirm with the DNO Discipline Supervisor(s) that the work activities are released for inspection prior to submission
of form. Ensure check box is ticked after confirmation and the DNO Supervisor is in email copy to the IR Form submission to QA/QC.
Note 3: IR number shall follow the following sequence IR- 001, IR- 002 etc for the particular project / scope.
Note 4: The issue of this document does not relieve the Contractor from its responsibility to provide a CLIENT compliant product or service.
Note 5: This Inspection Report is not a substitute nor stands as a formal report of any activity. It is provides and audit trail of compliances to the ITP.
Note 6: In circumstance were the work scope is split between two contractors a Work Transfer Certificate (WTC) May be required based on the work
criticality. This document is permanently embedded into this IR Form. Instructions for completion are provided within the WTC Form.
Note 7: DNO PM will notify both contractors formally with respect to the need WTC on a Project case by case basis.
