Step 1 - Sign-in Process

 Download IL TAKE CARE APP from Google Play store or Apple APP Store
 Go Directly to “Already have an account” and click Login
 Enter the Mobile number as per your HR Master then Click “Get OTP” and Enter the OTP
 If the mobile number not registered with policy , share me the mail with PS number and mobile

Step 2 – Policy Registration

Select Policies icon to see the current year GMC policy

4016/71639601/10/000

IF It’s not Linked , Pls click “ADD POLICY” in Main Page

and enter the details as below :

1. Select the option Policy Purchased by as “COMPANY”

2. Company Name : LARSEN & TOUBRO LIMITED
3. Employee ID : Enter the PS Number
4. Employee Name : Leave Blank or Delete
5. Date of Birth : As per EIP Records
6. And press “Add Health Policy”

Now you can see the policy got added in Policies Icon

Step 3 – Verify & Download E Card and Documents for Reimbursement Claims

1. Select Health policy from Main Screen – Valid Till 30-MAR-2023

2. Click in Self Card Details
3. Select Verify this card option and Verify all other cards for this policy option
4. Click Policies in Icon in Main Page and Select the policy 4016/71639601/10/000 and select the “Member Details” in it to
download E-Card

Documents for Reimbursement Claims :

1. ICICI Claim form completely filled(Patient's name UHID, Internal claim reference number etc.,)
2. EFT Form
3. Cancelled Cheque Leaf with Acc Holder's name printed
4. Final Bill with payment Receipts
5. Discharge Summary
6. Diagnostics report
7. Pharmacy Bills
8. Other Payment Receipts
9. Break up details, If package rate
10. Government KYC Documents*
11. RT-PCR Test Mandatory for Covid Claims
