AGREEMENT

regulations laid down by the Regional Cancer Centre, Thiruvananthapuram, pertaining to the
conduct of the Cancer Detection Camp/Cancer Awareness Programme/Training .

I hereby agree to abide by the rules and regulations pertaining to the conduct of Cancer
Detection Camp/Cancer Awareness Programme/Training laid down by the Regional Cancer
Centre, Thiruvananthapuram. I understand that the balance amount, if any has to be remitted
to RCC within 3 days after conducting the programme and I assure that I shall remit such
amount within this period.

REQUIREMENTS FOR CAMP

1. Two neat and clean examination rooms.

a. Doctor’s consultation room (spacious, well lighted (natural and artificial), well
ventilated.
i. One table and one chair for doctor.
ii. One examination table/desk.
iii. One stool and 1 chair for patient.
iv. Extra one table chair and two chairs.
b. Ladies examination room - maintains privacy, spacious, well lighted (artificial light)
and ventilated.
i. Close to doctors room, at least 12 X 10 feet size
ii. One gynaec- Examination table OR desk for cervical examination.
iii. One footstep.
iv. Two tables to keep articles.
v. Two chairs.
vi. Curtain / screen for separating cervical examination area and breast
examination area.
2. Waste basket – 2Nos. (Carton box).
3. Plug point (outside) for induction cooker.
4. Washing area for instruments with running tap water facility.
5. Generator / inverter to run light, fan and induction cooker in case of power failure.
6. Ice cubes in a flask / casserole.
7. Drinking water (1litre) - 2 bottles.
8. Hand wash- 1 bottle and towel – 2Nos.
