PROGNOSIS

Patient P had her prenatal visit at OB Clinic of
ONSET OF SPMC on March 26, 2020 accompanied by a
ILNESS social worker of DSWD. Patient P had her
regular OB check up till her admission on
October 25, 2020 with a complaint of
abdominal pain. She was diagnosed with
Pregnancy Uterine 38 4/7 weeks AOG G1P0.

Patient P was admitted on October 25,


DURATION OF 2020/02:40AM. She delivered her baby on
ILLNESS October 25, 2020/03:36AM and was
Discharged on October 29, 2020.

Patient P, a 15-year old orphan, is a victim of


PRECIPITATING sexual abuse. According to the patient's
FACTOR watcher who knew of Patient P's background,

 Patient P was heading home to the center


after school when her abuser forcefully carried
her on a dark secluded place where he took
advantage of her on February 14, 2020.
Patient P did not recognize the man who
raped her.

Patient P refused to take a bath on October


ATTITUDE AND 26, 2020. Thus, she is able to follow the

 She was from Surigao Del Norte, but was
abandoned by relatives after both her parents
died in a bombing incident thus she grew up
with other abandoned children at the
Department of Social and Welfare
Development (DSWD) since she was 3 years
old. Patient P has her watcher from the
DSWD to provide and assist for necessary
information.
Good: 3/5x100=60% Fair: 0/5x100=00% Poor: 2/5x100= 40%

Total = 100%
The highest score rate of Patient P’s prognosis is 60% for the “Good” category.

According to the record, the client has a positive attitude in medication treatment.and is

willing to take the process. She had undergone to the process far more better and fast.

Patient P had 40% for the “Bad”, for the hurtful reason of rape from a person she does

not even know, nor remembered the face. Being pregnant at the age of 15 is not the

ideal kind, and she has no one around aside from her DSWD famile since she was 3

years old. The overall prognosis of Patient P is under the “Good” condition.
