Gopole (Nurse 1): Good Morning Ma’am!


Lunar (Client 1): Good morning too

Gopole (Nurse 1): I am Jean Patrice Gopol I am from UPHSD and I will be you student
nurse Ma’am
Lunar (Client 1): Jean?

Gopole (Nurse 1): Yes Ma’am, I will be your student nurse, and I will be attending to your
concerns from Monday to Friday
Lunar (Client 1): Are you going to be looking after me?
Gopole (Nurse 1): Yes Ma’am I will be with you until Friday next week

Gopole (Nurse 1): Let me just confirm your name maam, are you Ma’am Myra Lunar?
Lunar (Client 1): Yes
Gopole (Nurse 1): How are you so far, Ma’am Myra
Lunar (Client 1): I’m good, the nurses here are very kind

Gopole (Nurse 1): Mmm. Have you eaten your breakfast, Ma’am?
Lunar (Client 1): Yes
Gopole (Nurse 1): May I know what you’ve eaten for breakfast, Ma’am?
Lunar (Client 1): I had some bread and ginger tea this morning
Gopole (Nurse 1): Wow, that sounds delicious Ma’am thank you for sharing. Do you have
any plans for today, Ma’am?
Lunar (Client 1): No, We can just talk
Gopole (Nurse 1): Sure, Ma’am what do you want to talk about?

Lunar (Client 1): just anything, feel free to ask


Gopole (Nurse 1): Where’s your husband, Ma’am?
Lunar (Client 1): He’s in San Francisco, he has a house there
Gopole (Nurse 1): *nodding* how about children, Ma’am, how many children do you have?
Lunar (Client 1): I have 2, both boys. The eldest is already 27 years old the other one
passed away when he was a year old

Gopole (Nurse 1): If it’s okay with you Ma’am may I know what is the cause of his death?
Lunar (Client 1): He just got a fever then a convulsion.
Gopole (Nurse 1): Have you tried bringing him to the hospital?
Lunar (Client 1): Actually, he wasn’t with me that time, when my husband and I got
separated, my eldest stayed with me, while my youngest stayed with him, that’s why I don’t
know if he was rushed to the hospital or not.
Gopole (Nurse 1): You don’t know Ma’am if your son was brought to a hospital?
Lunar (Client 1): I don’t know. My husband told me that he did, but i saw my son when he
was already in coffin
Gopole (Nurse 1): How did you feel during that time Ma’am?
Lunar (Client 1): I was in distress, I cannot stop crying that time

Gopole (Nurse 1): Thank you, Ma’am Myra for sharing your story. Our time is up, we’ll be
back tomorrow Ma’am.
Lunar (Client 1): Okay

(Lunar while walking back to the room will meet the patient 2 and nurse 2)
WORKING PHASE: NURSE 2 AND PATIENT 2
Geronimo (Nurse 2): Good Morning Ma’am. Are you ready for our activities for this
morning?
Gomez (Patient 2): Good Morning, yes yes. What shall we do today?
Geronimo (Nurse 2): Ah, we’re going to exercise then we can do some proverbs, and then
we can have a chat Ma’am
Gomez (Patient 2): Sure, okay…
Geronimo (Nurse 2): Ma’am what do you understand in the proverbs that says “the man
walking fast if pricked it would be deep”
Gomez (Patient 2): A man who isn’t cautious while walking is easily to be in danger
Geronimo (Nurse 2): People are likely to be involved in accidents if they are not careful in
their endeavours, is that what you mean Ma’am?
Gomez (Patient 2): Yes

Geronimo (Nurse 2): Are you satisfied with your snack, Ma’am?
Gomez (Patient 2): Yes, the bread is tasty because it has choco spread in it and I had some
juice
Geronimo (Nurse 2): How do you feel today Ma’am?
Gomez (Patient 2): I feel happy
Geronimo (Nurse 2): How are you earlier Ma’am? Did you have a good nap?
Gomez (Patient 2): I woke up early, it was 3 in the morning
Geronimo (Nurse 2): Do you usually wake up early Ma’am?
Gomez (Patient 2): yes

Geronimo (Nurse 2): May I know the reason Ma’am why do you usually wake up early?
Gomez (Patient 2): I don’t really know
Geronimo (Nurse 2): Do you have a lot of thoughts, Ma’am Kath?
Gomez (Patient 2): Yes

Gomez (Patient 2): Just my siblings, from time to time i usually think of them

think of them often?

Garcia (Nurse 3): Good morning Ma’am Precious
Precious (Patient 3): Good morning, what are we doing today?
Garcia (Nurse 3: We have a small party. We will do some exercise and then we’ll play, then
we’ll eat afterwards Ma’am.
Precious (Patient 3): Is there any choco milk and cornicks there?
Garcia (Nurse 3) Yes Ma’am. I brought 2 choco milks and a choco knot. You’re not yet
allowed Ma’am to eat cornick
Garcia (Nurse 3) : Ma’am this will be our last day here. We cannot join your socialisation
tomorrow. Would that be okay?
Precious (Patient 3): Why will you not be here tomorrow?
Garcia (Nurse 3): It’s a holiday ma’am, we don’t have duty tomorrow.

Garcia (Nurse 3): What will you do Ma’am since we’ll be out tomorrow?
Precious (Patient 3): I don’t know, but I’m sad since you’ll be gone.
Garcia (Nurse 3): *smiles* you should join the socialisation Ms. Precious, you’ll enjoy it, plus
you can win some prizes.
Precious (Patient 3): Okay, Where’s my choco milk?
Garcia (Nurse 3): Here Ma’am, Let’s go Ma’am we’ll accompany you to your room

Nario (Nurse 4): Good morning, I am Nurse Kevin and today I will be interviewing you for
assessment of your condition. (Nurse not smiling)
Moreno (Patient 5): Good day! Okay, nurse.
Nario (Nurse 4):Just answer as quickly as possible because I have other urgent cases
today, please.
Audrey (Patient 5): Oh, sure. I will try.

Nario (Nurse 4): Give me your name, age and where are you from Ma’am
Audrey (Patient 5): I’m Audrey, 25 years old and from Las Pinas.
(Nario Nurse 4):Okay. Got it.

(t: Client 4 here would pass by to these scene)


WORKING PHASE: NURSE 5 AND PATIENT 5
Pause 10 secs Orate (Nurse 5): As I can see in your chart, you have been developing
anxiety. When did your anxiety start?
Mabagos (Patient 5): uhm.. My anxiety started when I had my personal problems and
challenges,

Pause 10 secs Orate (Nurse 5): Do you live alone or with a family?
Mabagos (Patient 5): I was living with my father before, but due to all the things that are
happening to me. I have decided to get help from the nursing home, because I cannot take
care of him and myself.

Pause 10 secs Orate (Nurse 5): Oh, I see. If I were you, I would not put my father in that
situation… I guess older adults are always confused, right? And that’s when they need us
the most.
Mabagos (Patient 5):… *nodding or basta nakayuko slight lang naman*
Pause 20 secs Orate (Nurse 5): What are the other things that bothers you?
Mabagos (Patient 5): Well… uhm, I have been struggling on how to have a proper work-life
balance, and that’s what triggers my anxiety lately…

Orate (Nurse 5): I understand Sir, I can’t imagine having your father at the nursing home
while having struggles and confusion on your own. So I guess that’s why you are having
your anxieties lately.
Mabagos (Patient 5): I think so too, and I think putting my father in a nursing home will
somehow help me ease my anxieties a little bit. Because I know that he will have better
treatment there than here with me.
Orate (Nurse 5): Wait, Ma’am I’ll just call my colleague, I’ll be back

TERMINATION PHASE: NURSE 5, 7 AND PATIENT 6


Marentes (Nurse 6): You should have think twice before putting him there so that you will
not have anxiety. So for now I will record all of these as your assessment, Maam. I will
endorse you to our physician. —-
Pacis (Patient 6): But—
Orate (Nurse 7): Nurse Hashlee, Doctor Ferrer is looking for you, hurry up
Marentes (Nurse 6): Okay, I’ll be there. —--If you have any other questions Ma’am, just ask
okay? Please wait there for a while.
