 THERAPEUTIC COMMUNICATION IN ELDERLY PATIENTS
WITH HYPERTENSION

1. Pre-Interaction Phase
Three nurses will conduct an examination and see the progress of the condition of an
elderly patient named Mrs. Siti. Mrs. Siti suffers from hypertension who is being treated
in the jasmine room of the Mardi Waluyo Hospital, Blitar.

2. Orientation Phase
Nurse 1, Nurse 2, Nurse 3 came to the patient Mrs. Siti in the treatment room.
P1, P2, P3 : Assalamualaikum.
Family : Wa'alaikum greetings.
P1, P2, P3 : Good morning mother, Mrs. Siti (smiling)
Family : Good morning, nurse.

Grandpa was a little confused to see the arrival of the nurse.

P1 : Morning Grandma...!! How are you feeling today? Are you getting better?
Mr. Joko : Morning…!! Thank God it's been pretty good. By the way, how are you?
Grandma still looked confused and seemed to think..
P1 :Well, let me introduce my self, I am nurse Ayu, this is nurse Uli and this is
nurse Melin

Nurse 1, Nurse 2, Nurse 3 tried to approach Grandpa and also his family.
P3 : We are both in charge of taking care of you. By the way, have you had your
breakfast?
Mrs. Siti : Yes nurse, I have.
P3 : Did you eat a lot or not?
Mrs. Siti : Just a little bit because I don't have an appetite, Nurse. I still feel a bit
nauseous…!!
P3 : Ok , I see. Have you taken your medicine this morning?
Mrs. Siti : Well that's that then…!!
Family : Yes, Nurse, all the medicines have been taken...
After asking Grandpa, the nurse tried to explain the Nursing Care that would be given
to Grandpa and his family.
P2 : Alright Mrs. Siti. We’ll chcek your condition. Are you getting ready?
Family : Yes nurse, please do the best and help to get full recovery.
P2 : Yes ma'am thank you, we will try to do the best for your parents. We also ask
for your cooperation later in the investigation.
P2 : well, let us prepare for the equipment first and we’ll be back in 5 minutes.
Family : Yes nurse please..!!

After that the nurse left the patient's room to prepare the tools to be used in the action
to be given.

3. Working Phase
(Five minutes later, the nurse returns to the patient's room)
P1, P2, P3 : Assalamu'alaikum...
All : Wa'alaikum salam...
The nurse enters and immediately approaches the patient to take action.
P1 : Excuse me Mrs. Siti, please just lie down on the bed and relax.
Family : You can just lie down, mom.
After that the nurse immediately gave action to Grandpa.
P1 : Can you raise your hand, please?
(nurse 1 puts on a blood pressure cuff, then measures blood pressure)
P1 : By the way, how many grandchildren do you have?
Mrs. Siti : Eeehm,, 3 nurse. They already growing up.
P1 : Ooh, i see. Are they married?
Mrs. Siti : The eldest is married and others still in college. They are very good
grandchildren.
P1 : It must be, like you.
While waiting for nurse 1 to measure blood pressure, nurse 2 prepared a thermometer
to take Grandpa's temperature.
P3 : Now, let me take yor temperature. Can you lift up your right hand?
Mrs. Siti : Ok, nurse.
P3 : Good, let me put the thermometet an wait for a while.
Mrs. Siti : Yes, nurse. I see.
After a few minutes later the blood pressure and temperature had been measured, then
the equipment was removed again, and after that nurse 1, nurse 2, nurse 3 continued to
check her pulse and breathing.
P2 : Alright, Mrs. Siti. Now, I’ll take your pulse and respiration rate. Can you give
me your index finger?
Mrs. Siti : Ohh, I see. Here it is, nusre.
P2 : Ok, let me put on the oximeter. It’s done.

4. Termination phase
After all the examinations have been carried out, the results of the examinations are
recorded by the nurse and all equipment is tidied up
Family : Excuse me, nurse. How about the progress of my parents? Is it improved?
P1 : Well, she has very significant progress, sir. But, she needs to drink a lot of
water and eat more vegetables
P2 : Yes, she is right. Your mom also needs lots of rest and be relaxed.
Family : Alhamdulilah, that's really good news, nurse
P3 : Alright, sir. We have to visit another patient now. I hope you mother will be
discharged soon. If you need help, you can find us in the nurse station.
Family : Thank you so much nurse for your help.
