I. Project Title: PROJECT INNOVATE

II. Situation Analysis:

As the COVID-19 pandemic continues, recording parents/guardians


and students’ attendance during the distribution and retrieval of modules is
risky for the health of students, teachers, parents/guardians, and other school
personnel.
In this regard, the school launches” PROJECT INNOVATE to make
attendance recording method that is helpful in keeping track of students who
already has a copy of modules every week without risking the health of
teachers, students, parents/guardians and other school personnel during the
distribution and retrieval of modules. This project will be part in the safety
protocols for covid 19 prevention that they need to follow.

III. Rationale:
Despite the situation in teaching and learning delivery cause by the
pandemic, the DepEd policy “Education for All” should always be taken to
consider especially in the learning modality we offer this time of pandemic.
That is why modular distance learning using printed modules is still in our
learning modality as part of our blended learning modality to reach those who
do not have internet access and even mobile phone to keep posted in
learning activities despite the high risk of exposure to Covid 19 Virus.
In accordance with the guidelines of Department of Health and IATF
both in the national and local government that needed to follow for COVID 19
Preventions in public schools we come up with the Project Innovate as a
means of recording attendance for distribution and retrieval of modules to
minimize the risk of getting exposed to Covid 19 Virus.

IV. Goals and Objectives:

The following are the targets of the proposes project:

1.Organize a team to be led by the ICT coordinator.

2. Provide an attendance recording method that will minimize the risk of getting the
covid 19 virus.
 
3. Educate the classroom advisers with the project through SLAC/INSET.
 
4. Assist the class advisers in producing Leaner’s ID cards by means of creating a
template for the ID’s.

5. Include scan QR Codes of students as part of the procedure during the


distribution and retrieval of modules.

VI. Expected Outcomes:


Learner’s ID with QR Code that will be scan by the adviser with the help of
QR Code Scanner application downloaded on their mobile phones during the
distribution and retrieval of modules as method of recording attendance to

minimize contact with the guardians/parents as a means of protocol for covid

VII. Resources Requirements:


Funds to be used in the said project will be taken from school funds.

VIII. Operational Plan:


The Project will be initiated by the school in partnership with the ICT Team

IX. Monitoring and Evaluation:

The Project was initiated by the school to be facilitated by ICT Team in


coordination with the advisers.

X. Project Proponent:

ICT Team of this school will initiate the said project to be participated by
the all Classroom Advisers.

XI. Project Cost:

To be assessed by the Project Team of the school. Included all the


materials, and supplies to be used in the project.
