one of the heirs of Mr. PETRONILO YANGCO RIVERA most respectfully
inform the management that PETRONILO YANGCO RIVERA one of your
bank depositor with Bank Account Nos. 086-3-08621514-5 and 086-7-
08600151-5 died on February 5, 2022 in TRILLUM HEALTH PARTNERS,
MISSISSAUGA ONTARIO, CANADA, photocopy of STATEMENT OF DEATH and
Medical Certificate Form 16 are hereto attached as ANNEXES “A”
and “B” respectively. After the death of PETRONILO YANGCO RIVERA
his heirs namely:

1. ELISA RIVERA ASERON,

2. TIMOTHY ASERON,
3. DANILO HIZON ASERON,
4. DIANA RIVERA O’KEEFE and
5. AARON O’KEEFE

Executed a DEED EXTRAJUDICIAL SETTLEMENT OF ESTATE WITH WAIVER OF

RIGHTS subscribed before JOSEF-JAKE CAMACHO AGUILAR, A Notary
Public for the Province of Ontario, photocopy of DEED
EXTRAJUDICIAL-SETTLEMENT OF ESTATE WITH WAIVER OF RIGHTS is
hereto attached as ANNEX “C”. In the said document all the above-
named heirs of Petronilo Yangco Rivera waived their rights over
the property left by the deceased Petronilo Yangco Rivera in
favor of DANILO HIZON RIVERA.

IN VIEW OF THE FOREGOING CIRCUMSTANCES AND FACTS attendant to the

demise of Petronilo Yangco Rivera and based on the DEED
EXTRAJUDICIAL SETTLEMENT OF ESTATE WITH WAIVER OF RIGHTS, Mr.
Danilo Yangco Rivera respectfully request for the closure of Bank
Account Nos. 086-3-08621514-5 and 086-7-08600151-5 in the name of
Petronilo Yangco Rivera and that all amounts of money deposited
in these two (2) Bank Accounts be released and/or remitted to Mr.
Danilo Hizon Rivera at his address at 71 Addington Cres,
Brampton, Ontario, L6T 2R4, Canada.
