RE: Collingwood Collegiate Institute Prom

CCI Students who are currently enrolled in their 4th or 5th year of school (Grade 12) are invited to attend
Prom 2022. Each student is also eligible to bring one guest. A guest’s ticket must be purchased at the
same time as the CCI student purchases his/her ticket. This is a school sanctioned event planned by the
2021-2022 Prom Committee and the school’s code of conduct will be in effect. In order to participate in
this activity, students must have paid their student activity fee for the 2021-2022 year, have fewer than
10 unexplained absences in the current semester, have completed their 20 community service hours,
and they must not have any property owing to the school.

The Prom Committee has worked very hard to offer exceptional value while keeping the price as low as
possible. The price of the ticket is $65/person which includes a full buffet meal, unlimited fountain
drinks, lots of fun awards, a dance , a photo booth and memories to last a lifetime. Prom will take place
at Craigleith Ski Club, 164 Craigleith Road, Town of the Blue Mountains, on Friday, May 20th, 2022 from
5:00 p.m. to 10:00 p.m.

The school’s responsibility for the student begins when the student arrives at Craigleith Ski Club (doors
open at 5:00 p.m. and students must arrive before 5:45 pm) and ends when the student leaves
Craigleith Ski Club. The event ends at 10:00 p.m. Any activities that take place before and after prom
are not school sanctioned or supervised, and students who participate will be doing so at their own
risk.

This promises to be an exciting evening for our senior students. By working collaboratively with
parents, guardians, and students, we can make this a safe and fun evening for everyone.

INFORMATION FORM:
There is an information form that we require you to sign, which outlines the CCI Code of Conduct and sanctions in
effect to ensure the safety of each person in attendance at the prom. A guest, who is not a student of CCI, is the
responsibility of the CCI student and his/her emergency contact. This guest will have a form to complete requiring
endorsement from their school. A separate information form can be completed for a guest who is a CCI student. A
ticket can be purchased once the information forms are completed in full and returned. Tickets will be for sale in
the cafeteria at the first half of lunch. Tickets will be sold starting Wednesday, April 20, 2022, and will continue to be sold
as long as tickets are available, however the final date to purchase a ticket is Friday, May 5, 2022, if any remain
until 3:30. This will be the last opportunity to purchase a ticket (NO EXCEPTIONS), as our caterers require final
numbers at that time.

❖ The ticket price is non-refundable.
❖ Tickets can be purchased once this form is signed and returned completed, as long as tickets are still for
sale/available.
❖ Tickets are not transferable between students.
❖ If any remain, the last day to purchase tickets is before 3:30pm on Friday, May 5, 2022.
