FIA Spring Formal 2022 Permission Form

Dear FIA Families and Friends,

We are very much looking forward to FIA’s first Spring Formal, scheduled for
this Friday, April 1. As our FIA students are entitled to bring one guest, this
form is required of the guest and his/her parents, in order to be allowed to
attend the dance, which will be held in Reading at The Wharf House. Chaperones
for the evening will be sta from the FIA high school campus; security will be
provided by Guardsman/Beryllium.
