AFFIDAVIT OF BEING SINGLE

I, ELOISA JANE SABILES ANACION, of legal age, single, Filipino citizen and
with residential address at Barangay Gomez, Lopez, Quezon, after having been sworn
to in accordance with law, hereby depose and state;

That I am applying in the Military Service as trainee in the

Philippine Army;

That I was born on June 05, 2000 and one of the children of
spouses Ernesto E. Anacion and Marivic S. Anacion;

That I am still single, as I have never contracted any marriage with

another person up to the present;

That I am executing this affidavit to attest to the truth of the above-

statement for whatever legal purposes that this affidavit it may serve.

I, ELOISA JANE SABILES ANACION, of legal age, single, Filipino citizen and

That I am applying in the Military Service as trainee in the

Philippine Army;

spouses Ernesto E. Anacion and Marivic S. Anacion;

as Annex “A” and made an integral part hereof]

That I am still single, as I have never contracted any marriage with

another person up to the present, and I remain single to this very day and
hour;

[Photocopy of Affidavit of Being Single is attached herein as Annex

“B” and made an integral part hereof]

That, I have no children whether legitimate or illegitimate;

Accordingly, I am hereby declaring spouses Ernesto E. Anacion

and Marivic S. Anacion, my biological parents, as my legal beneficiaries in
all insurance that will be due to me in all pertinent documents that will
need the declaration of legela beneficiaries;

That I am executing this affidavit to attest to the truth of the above-

statement for whatever legal purposes that this affidavit it may serve.
