1. PURPOSE
The purpose of this document is to provide comprehensive guideline and procedure for
activities related to lifting, shifting, stringing, lowering-In, transporting pipeline, materials and
equipment, fit up and tie in welding using excavator with suitable load at GRAPHITE Project.
Another purpose of this document is to set up a procedure and safety precautions that shall
be considered during lifting works using excavator, to enable all related operations done safely
without risk to personnel and assets and comply with minimum legal requirements.
2. SCOPE
This scope of this document includes the minimum requirement for lifting, shifting, stringing,
lowering-in and transporting material and equipment using excavator.
Definitions:
a. Lifting
Lifting of object using excavator.
b. Shifting
Lifting and moving of object by using excavator arm in static position.
c. Stringing
Stringing of line pipes by using excavator.
d. Lowering
Lowering of object from higher position to a lower position.
e. Transporting
Moving or transferring of object from one place to another by using excavator.
The detail of work scope between all parties involved shall be confirmed prior to
commencement of work at site, as follows:

4. RESPONSIBILITY
Project Manager
Shall report to the Project Director and shall be overall responsible for the project
including providing adequate resources with quality and safety.
a. Provide adequate resources and information for the planning and execution of all lifting
operations
b. Ensure that established standards and procedures related to lifting and hoisting
operations area available and implemented in accordance with applicable regulations
and recognized industry standards.
c. Provide proper training in safe operation of lifting and hoisting equipment for its
relevant personnel.
d. Adequately provide the required personal protective equipment (PPE) as identified in
the risk assessment.
Construction Manager
Shall report to the Project Manager, shall be responsible for construction activities
related to this procedure, and shall coordinate with the Project Planning, QAQC and site
engineer/supervisor for implementation of this procedure.
a. Identify key personnel for the control, supervision, and execution of Lifting, Shifting,
Stringing, Lowering-In and Transportation Using Excavator activities.
b. Control requirements to ensure safe execution.
c. Resources including manpower, machinery and equipment for the activities are being
made available.

d. Make sure inspection and maintenance programs for all equipment & machineries
required for Lifting, Shifting, Stringing, Lowering-In and Transportation Using
Excavator activities are conducted and recorded.
e. Ensure all non-compliant machineries are not being utilized at Site.
f. Ensure adequate resources for the works.
g. Ensure all machineries are fit and in good condition.

QAQC Manager
Shall report to Project Manager. Shall be overall responsible for QAQC related matters
and inspections .
QAQC Inspector
Shall report to the QAQC Manager. Shall be responsible for control and inspection of
QC activities.
Construction Engineer
Shall report to Construction Manager. Construction Engineer shall be responsible for the
overall site execution which includes but not limited to preparation of documentation and
compilation of reports and ensure resources are adequate and competent. Construction
Engineer is also responsible to ensure the scope of work at the specific location is safely
executed and completed within specified schedule and compliance with technical and quality
requirement.
Construction Engineer is to ensure the lifting weights (loads) are being thoroughly reviewed
and discussed with Lifting Supervisor.
Construction Engineer shall be responsible to prepare lifting plan for site lifting operations
and ensure all personnel involved are adequately briefed on the plan to enable all
activities are carried out safely.
Lifting Supervisor
Shall report to the Construction Engineer. A Lifting Supervisor is defined as the person in
charge who actively supervising the lifting operation on site.
a. Develop the lift plan with supervision of construction engineer and coordinate all lifting
operations to ensure that the lift is carried out in a safe manner in accordance with the
lift plan.
b. Verify that all personnel are trained and competent to safely perform lifting operations.
c. Verify that lifting equipment pre-use inspection has been conducted and that lifting
equipment are in good mechanical and operating condition.
d. Verify that all relevant lifting certificates/documents are valid, and the necessary
approvals obtained.
e. Conduct pre-lift safety briefing for all personnel involved to communicate the lift plan,
safe lifting procedures and risk assessment.
f. Lifting supervisors shall be properly trained and qualified to the level of lifting
operations using excavator under his supervision.
g. Lifting supervisors to be full-time at Site during lifting activities and provide signal for
lifting activities.
h. Lifting supervisors must be certified with approved 3rd party and attended rigging and
slinging course.
i. Lifting supervisors to join the inspection with mechanical inspector and HSE rep for
the lifting equipment.

Training for lifting supervisors shall address relevant lifting standards and legislation, safety
inspections and safe lifting operations.
Site Superintendent
Shall report to the Construction Engineer and shall be responsible for safe performance
of the activities at Site.
Helper
Shall report to the Site Supervisor. The helper is responsible for performing all works
ordered by supervisor correctly and safely. They are mainly responsible for performing all
works at site or any tasks as per instructed by Work Leader and other superior from time to
time. Work Leader to follow safety procedures when performing the works.
Excavator Operator
Shall report to the Site Supervisor. Excavator Operator is responsible to operate excavator
correctly in a safe manner and practice. The operator shall be certified and have adequate
knowledge about the soil, terrain and the load condition to avoid the excavator getting
stranded or topple at site and shall be alert on the communication with lifting supervisor.
Excavator operator shall be trained and certified by training provider recognized by CIDB.
a. All slings, webbings, shackles, and other lifting accessories used to rig, the loads are
within the SWL of the lift, in good condition and duly certified.
b. Load is rigged up in such a manner that it is stable, balanced (centre of gravity below
the hook) and secured (i.e., no loose items).

c. The lifting appliance operator has been informed of the weight of the load. Use tag
lines to control the load as it is being maneuvered. Report any defects in the
lifting/rigging equipment to the lifting supervisor.
5. HSE REQUIREMENTS
a. CONTRACTOR to perform lifting plan review and safety briefing a day prior the lifting,
shifting, stringing, lowering-in and transporting using excavator operation. The
excavator to be utilized at Flat Terrain area only.
b. Specific load chart for the excavator to be posted in the excavator for quick reference.
c. A site-based traffic management plan should be in place. Traffic management plan for
JKR’s road crossings is in place. (GRAPHITE-EXE-MMSJV-KTHRO-HSE-PLN-007).
d. Heavy machinery and vehicle roadways should be separated from pedestrian walkways
with physical barriers.
e. The layout of the site should be designed to minimize the need for reversing of heavy
machinery.
f. Reversing of heavy machinery should not be the normal operating mode as it is
dangerous.
g. Engines should be shut off during re-fueling.
h. Non-machinery operators should be trained to work safely around the machine and
wearing high visibility clothing.
i. The traffic controller shall always stand in a safe area at all times when the heavy
machinery is moving and have constant communication with operator.
j. Ensure adequate number of signalmen and wearing reflective safety vests or reflective
jacket safety.
k. Passengers are NOT permitted to ride any heavy machinery.
l. Mobile phones, whether hands free or not, should only be used by the operator while
the machinery is stationary.
m. Operator are trained to operate at hazardous terrain or slopes where applicable.
n. Operators should apply 3-point contact at all time rule when ascending or descending
the heavy machinery.
o. All equipment, inclusive machinery, appliances and shifting gears shall be visually
examined by trained personnel.
p. All equipment shall have a valid certificate.
q. The safety system and safety devices shall be in proper functioning order.
r. The weight of the load shall be established and calculated to ensure it does not exceed
safe working load of the machinery.
s. Lifting, shifting, stringing, lowering-in and transporting using excavator operation shall
only be conducted during fine weather i.e., no heavy rains or lightning. CONTRACTOR
shall take into consideration the weather forecast before commencing work. For lifting,
shifting and transporting works, each excavator (Only EC300 or equivalent) to
handle/transfer one line pipe only (maximum).
t. CONTRACTOR shall have an experienced supervisor to do site assessment prior to job
execution by implementing flagging system.
u. CONTRACTOR shall ensure stable working platform for excavator positioning on site.
v. To conduct a soil movement monitoring via soil pegging at pre-identified site.

w. The excavator operator shall not move the machine until rigger, or any other persons
associated with the works have all moved away from the danger area. Only authorized
personnel shall be allowed to enter working area.
x. The excavator operator shall not leave the operating controls while the loads is
suspended.
y. The lifting activity using excavator shall only practice at site condition in which there
shall be no other way for crane / mobile crane / side boom to be operating.
z. Distance track excavator from edge of the trench minimum 1 meter.
aa. Operation of Heavy Machinery shall be managed in reference to PTG 18.55.06 –
Operation of Heavy Machineries, revision August 2021.
5.2 Trainings
All personnel involved in lifting operations shall receive training which covers safe lifting
procedures and technical aspects at various levels depending on their competency
requirement. The training and competencies stated in this section should be complementary
for the required training/competencies that are imposed by the Government and the
Regulatory Bodies. Only trained personnel shall be allowed to work during the lifting, shifting,
stringing, lowering-in and transporting using excavator activities. The trainings should include
but not limited to:
Excavator Operators and respective personnel shall attend safe operation training by certified
COMPANY expert for performing lifting, shifting, stringing, lowering-in and transporting using
excavator activities. In house training to be conducted and the training must meet the
requirements set by Construction Industry Development Board (CIDB).
CONTRACTOR shall include practical section or a class (trainer to trainee) and reading of Safe
Load Table in the training module. The training module will be reviewed and approved by
COMPANY prior to implementation. Refresher for the training to be done after 3 years.

CONTRACTOR shall mobilize the following machineries to safely perform the work as per
lifting certificate;

An effective and safe excavator is one which is well matched to the type and size of load to
be carried and lifted, together with the environment in which it is to be used.

The selection process of excavator inclusive the size should consider the following points:
Lifting equipment can be categorized into three sub-groups which are lifting appliance, lifting
accessories and lifted equipment. The lifting equipment shall include but not limited to list as
below as per stated in PTS 18.52.06 – Lifting And Hoisting.

CONTRACTOR to ensure that SWL & Safety Factor requirement for all lifting equipment and
accessories are considered.
CONTRACTOR to also ensure that all lifting belt is color coded as per PETRONAS color coding
and are clearly marked with equipment identification number, SWL and inspection date expiry.
All lifting gear/ equipment shall be color coded for visual compliance as follows:
All machineries and equipment’s used in the work of lifting, shifting, stringing, lowering-in
and transporting using excavator of pipeline materials and equipment using excavator shall
follow the Government regulations and requirements as well as PETRONAS Technical
Standards (PTS).
No modification is allowed for excavator. CONTRACTOR to submit the modification design and
calculation to supplier/manufacturer for acceptance.
DOSH shall be notified on the usage of excavator for lifting, together with any modification
for construction.

The existing excavator manufactured bucket hook shall be changed to a pad eye. The safety
factor for engineering design & calculations for the modification shall be assigned as 2. The
Pad Eye design calculation is attached in Attachment 9.
Welding pad eye with bucket excavator shall be tested (MPI) to make sure no cracks or
damages during welding and load tested. Regular MPI will also been conducted to check the
condition of the Pad Eye from time to time. Shackles to be removed if excavators are not
being used for lifting.
Load test to follow the PTS 18.52.06 – Lifting And Hoisting requirements which must be at
least twice the weight of the required load. Criteria of the excavator load test are as per
below;
5.5 Communication
The primary communication system on site between Work Leader, Excavator Operator and
Lifting Supervisor shall be using the hand signal.
Only common language (hand signals to be used) that has been tested and agreed by Work
Leader, Excavator Operator and Lifting Supervisor will be used for communication during
works.
Work Leader, Excavator Operator and Lifting Supervisor to also utilize personal mobile phone
or walkie talkie for communication, if required.
6. PROCEDURE
COMPANY and CONTRACTOR have legal obligations to plan and control the operation of
excavators on site. Operators and personnel on Site must be made aware of site hazards
which could affect the safe use of excavators and of site rules which limit where they may be
used.

• Vehicle only areas, especially where space is limited, or traffic is heavy; and
• Safe vehicle routes around the site.
When planning the movement of vehicles on site, including pick and carry routes, access
to the lifting zone should be carefully controlled to ensure that personnel not associated
with the lifting operation are excluded to reduce the risk of contact with the excavator or
load being lifted.
This procedure is applied at identified area of the ROW and shall include but not limited
to Attachment 1 for mapping area for lifting, shifting, stringing, lowering-in and
transportation using excavator.
b. Site Housekeeping
Areas where an excavator may be used should be kept free of rubbish such as bricks,
pallets, timber, etc. Such debris can significantly affect the safe operation of an excavator
when lifting suspended loads.
d. Ground Conditions
The Construction Engineers and Lifting Supervisor shall refer to (Attachment 12 –
Underground Pipe Stress Calculation) for the excavator loads can be safely supported by
the ground. Similar consideration must be given where excavators are sited on suspended
slabs or positioned near retaining walls or adjacent to excavations or embankments, and
steep area.
The ability of the ground to support the excavator will depend upon the type of
excavation, the nature of the ground and ground water conditions. The condition of spoil
heaps also requires careful monitoring as they can be adversely affected by rain or wet
weather.
The excavator position must be adequately strong, firm and level on ground. Devices,
such as steel plates or construction mats, etc. must be provided to spread the load from
the excavator to the ground whichever required to fully ensure the working platform is
stable ground and safe for the throughout the lifting, shifting, stringing, lowering-in and
transporting using excavator.
e. Underground Services
The area where the lifting operation is to take place, including any pick and carry route,
should be assessed to ensure that any underground services are located, and measures
put in place to ensure that the services are not damaged and that they do not affect the
stability of the excavator.

The minimum extent of these zones varies according to the voltage of the line, as follows:
• low-voltage line – 1 m;
• 275/132 kV line at KP3 (tower mounted) – 7 m.
The minimum heights above ground level for overhead power lines and can be used for
the identification of line voltage from the type of pole or pylon.
Goal post with height signage to be set-up at the access of the area to reduce the working
envelope of the excavator.
Greenfield and brownfield shall be marked and pegged by Surveyor. Greenfield and
brownfield area to be demarcated by using physical barrier (i.e: Snow fence).
Safe Load Calculations to adhere to the manufacturer’s manual. The safe working load table
is varied depending on the type of excavators, refer Attachment 2 and Attachment 3.
CONTRACTOR to ensure trained personnel are available to read the Safe Load Table.
Daily, Weekly and Monthly inspections are required to be conducted. Pre-use daily inspection
shall be conducted at the beginning of each shift, prior to lifting, shifting, stringing, lowering-
in and transporting using excavator into service, and during extended operation (when
necessary) to ensure its serviceability and integrity.

Any damaged or unfit lifting equipment shall be withdrawn from services and sent for
adjustment, repair, or replacement.

6.5 Lifting, Shifting, Stringing, and Transporting Using Excavator During Execution
a. Minimum usage of 6 units of class 200/210 excavators to be used for lowering activity
with a span of 12m. Refer Attachment 10 for the diagram arrangement. For this type of
machinery arrangement, the 6 units of class 200/210 excavators must be in a front-
loading position for the lowering-in works. Also, for this type of lowering activity, the
class 200/210 excavators are preferred to be with the same brand from the same
manufacturer to ensure similar load capacities.
b. Another option is to utilize 4 units of class (300/330/350) excavators – with a span of
24m from each other and 2 units of class (200/210) excavators at each end – with a
span of 12m from the closest excavator. Refer Attachment 10 for the diagram
arrangement. For this type of machinery arrangement, the 6 units of excavators will be
in a side-loading position for the lowering-in works. Both excavators’ class (200/210) at
each end will not only be as a guide to the lowering activities, in addition it shall carry
50% of the load (equivalent to 1.64T from 3.28T) at each end.
c. Distance track excavator from edge of the trench minimum 1 meter.
d. Load distribution during lowering-in, refer to Attachment 7 and Attachment 8 for the
Calculation for Pipeline Free Span of Lowering Activities.
e. CONTRACTOR shall identify the suitable location for the position of the excavator.
f. CONTRACTOR shall ensure the safe distance to the object for the load/unload location.
The weight of the object must be known, and it shall be referred to the excavator load
chart to check for the safe reach boom lift capacities.

g. Load chart and the JMS will be included in the excavator cabin for quick reference.
h. Webbing sling with shackle (if required) shall be secured to the object and a tagline shall
be tied to the object for controlling or to prevent from swinging during lowering
operation.
i. While lowering-in using excavator, the lifting supervisor must guide the excavator
operator and to ensure that no personnel entering under the suspended load.
j. Once the object has reached the location, the excavator operator shall slowly bring down
the object to the ground and rigger shall remove the shackle and sling from the object.
k. Once the lowering-in work has completed, excavator shall be parked at the safe place.
l. Lifting, shifting, stringing, and transporting using excavator activity shall be immediately
stop during heavy rain or strong wind.
m. Night-time activities for lowering-in using excavator shall be performed with sufficient
lighting. LUX test to be conducted at all applicable locations (access and working area).
Acceptable lux is 100 lux for working area and 50 lux for access area.
6.7 Lowering into Trench Using Excavators and Side Booms Arrangement
a. 6 nos of machineries to be utilized for the works – 2 units of class 200/210 excavators
to be used at each end, followed by 2 units of class (300/330/350) excavators and 2
units of side booms in the middle of the pack. All machineries will be within a span of
24m.
b. For this type of machinery arrangement, the machinery will be in a side-loading position
for the lowering-in works.
c. Refer Attachment 10 for the arrangement diagram.
d. Distance track excavator from edge of the trench minimum 1 meter.
e. Load distribution during lowering-in, refer to Attachment 7 and Attachment 8 for the
Calculation for Pipeline Free Span of Lowering Activities.
f. CONTRACTOR shall identify the suitable location for the position of the excavator and
side boom.
g. CONTRACTOR shall ensure the safe distance to the object for the load/unload location.
The weight of the object must be known, and it shall be referred to the excavator and
side boom load chart to check for the safe reach boom lift capacities.
h. Load chart and the JMS will be included in the excavator and side boom cabin for quick
reference.
i. Webbing sling with shackle (if required) shall be secured to the object and a tagline shall
be tied to the object for controlling or to prevent from swinging during lowering
operation.
j. While lowering-in using excavator and side booms, the lifting supervisor must guide the
excavator and side boom operator and to ensure that no personnel entering under the
suspended load.
k. Once the object has reached the location, the excavator and side boom operator shall
slowly bring down the object to the ground and rigger shall remove the shackle and sling
from the object.
l. Once the lowering-in work has completed, excavator and side boom shall be parked at
the safe place.
m. Lifting, shifting, stringing, and transporting using excavator and side boom activity shall
be immediately stop during heavy rain or strong wind.

n. Night-time activities for lowering-in using excavator and side boom shall be performed
with sufficient lighting. LUX test to be conducted at all applicable locations (access and

6.8 Fit up and Tie-in Welding Using Excavator and Side Boom
Lifting certificate will be applied during PTW applications and shall follow standard lifting
requirements of the Project. Refer lifting plan for excavator and example of calculation of
lifting certificate as per Attachment 5.
