Remarks on site condition: Hot weather with humidity. The work location was okay due to sufficient number of
ventilation fans.
Description of activities:
1. Drilling of anchor points for hangers.
2. Installation of hangers.
3. Erection and dismantling of scaffolds.
4. Shifting of materials from store area to work area.
5. Hot works: cutting and grinding of steel rods.
6. Installation of portacabin for site office (installation of electrical and plumbing fixt).

1. The site store not properly ordered.
2. Poor lighting in the site store area.
3. Erection and dismantling of scaffolds carried out by unqualified/uncertified personnel.
4. Poor housekeeping; HSE officers have to raise issues every day before housekeeping is done
5. Colour code tapes (GREEN for now) urgently needed, we must colour code all electrical and power tools
before use on site. Our grace period is over.

Any delays and reasons:


There was power failure on 30th of July on corridor 3. It was PSH responsibility; PSH electrician fixed it ASAP.
Items for office action: The following items are required on site to ensure
compliance with site safety requirements:
1)Lighting for site store area. 2) Wooden box with lock (for keeping first aid box inside corridor 3.) 3)
Heat Sprays for first aid 4) Safety shoes for Labours 5) Heat stress meter 6) Safety jacket 7)
Safety Helmet (Blue) 8) Dust masks (P95) 9) Dark glasses 10)Ear plugs 11) Wooden housing for fire
extinguishers
Closing Remarks:
1. Water bottles supplied to site for workers.
2. Heat stress meter is needed for the HSE department so that we can adequately monitor site conditions viz
temperature and humidity.
3. Certified scaffold erectors should be deployed to the site to handle erection and dismantling of our
scaffolds. A competent scaffold inspector is also required on site for the inspection of our scaffolds.
4. Office portacabin almost completed.
5. The 2 scaffolds without toe-boards dismantled pending when the vendor provides the toe-boards. Only 2
scaffolds in use now.
6. Saturday TBT now to be recorded only on PSH sheet. Directive from PSH safety manager.
7. Medical test is now compulsory for all workers on site. It is a prerequisite for site induction.
