JOB ORDER CONTRACT

Makataktak Elementary School represented by the Head Teacher AGUSTIN C.

BACUDO JR. referred here as the first party and REX A. CLAUD referred here as the second
party entered into this JOB ORDER CONTRACT with the following conditions to wit:

That the FIRST PARTY will hire the services of the SECOND PARTY to do the
following job to wit:

1. Labor: Expansion of school concrete perimeter fence

That the FIRST PARTY will pay the amount of seven thousand , two hundred
eighty nine pesos and ten centavos. (PhP 7,289.10) to the SECOND PARTY as payment for the
said contract.

That the SECOND PARTY will start their work on December 1, 2021 until
December 14, 2021.

In witness whereof, have here to set their hands this 8th day of October 2021 at
Makataktak Elementary School, Makataktak, Liliongan, Carmen, Cotabato.

BACUDO JR. referred here as the first party and GERRY A. LEGADA, referred here as the second
party entered into this JOB ORDER CONTRACT with the following conditions to wit:

That the FIRST PARTY will hire the services of the SECOND PARTY to do the
following job to wit:

1. Labor: PUTTING UP OF CANOPY AND INSTALLATION OF WATER CONNECTIONS IN

HANDWASHING FACILITY (3000.00)
2. Labor: PAINTING OF ROOF (2,755.50)

That the FIRST PARTY will pay the amount of five thousand seven hundred fifty
five pesos and fifty centavos(5,755.50) to the SECOND PARTY as payment for the said contract.

That the SECOND PARTY will start their work on March 25, 2021 until April 13,
2021.

In witness whereof, have here to set their hands this 22 nd day of March 2021 at
Makataktak Elementary School, Makataktak, Liliongan, Carmen, Cotabato.
