I, REYNALDO O. SOTERO, legal age, married, Filipino citizen and with
residence and postal address at Daclan, Tublay, Benguet after having been
duly sworn to in accordance with law hereby depose and state that:

1. I am a member of the Sanguniang Bayan of the Municipality of

Tublay, Province of Benguet being a Municipal Councilor;

2. I am applying for Terminal Leave Benefits and among the

requirements is affidavit of no pending criminal investigation or
prosecution;

3. In compliance therewith, I hereby undertake and states that I have no

pending criminal investigation or prosecution or case/s, either
criminal, civil or administrative cases, filed against me nor been
charged of any act defined and penalized under RA No. 3019 or the
Anti-Graft and Corrupt Practices Act;

4. I am executing this affidavit to attest to the veracity and truthfulness

of the foregoing statements and for all legal intents and purposes it
may serve.
